ADDENDUM "D"
GENERAL AND SPECIFIC RULES OF PROBATION
The following provides a sample of the standard document executed
by persons admitted to probation in the Washington Court House
Municipal Court.
The terms and conditions are subject to
amendment as directed by the Presiding Judge.
READ CAREFULLY BEFORE SIGNING
"You are in custody of the Court by reason of your misdemeanor
conviction.
Your rights are restricted until your probation is
terminated.
As a probationer, you can be arrested without a
warrant by any Probation Officer or any other police authority,
whenever there are reasonable grounds to believe that you have
violated the terms of your probation."
Probation is for a period of
years.
GENERAL CONDITIONS THAT APPLY TO ALL PROBATIONERS
1.I will avoid trouble and places where the law is violated by
others.
2.You shall contact the Probation Officer within 48 hours of an
arrest, or questioning by any law enforcement officer.
3.I will support those persons whom I am required by law to
maintain support, and seek a job if unemployed.
4.Must obey all Court orders issued by this Court or any other
court.
5.Must not use abusive language to court personnel.
6.Must not violate any State laws or City and/or Village
ordinances.
7.Must be on good behavior and act accordingly during term of
probation.
8.Are subject to a random warrantless search of your person, motor
vehicle, place of residence, or any area under your immediate
control, by your probation officer or any local police
officer or deputy sheriff, at any time during the period of
your probation.
The Court may fix additional specific terms of probation, if
they apply to your case.
These terms are also a direct Court
order and you will be required to follow them.
SPECIFIC CONDITIONS OF PROBATION
9.Must pay fine and cost as per agreement.
10.Must make restitution for damages and/or medical bills.
11.Must enter and complete alcohol and/or domestic violence
counseling at his/her cost.
12.I will first seek permission from my Probation Officer before
leaving the county or state, and will immediately report any
change in my home residence, and also any change of
employment.
13.I shall report to my Probation Officer as instructed and as
often as required by him or the Court.

14.When and if limited driving privileges are granted, defendant
must comply with any conditions of the driving permit and
operate his vehicle within the scope of the permit.
15.Must perform public service work for

alcohol.
21.Must not consume/possess any alcoholic beverage or any illegal
drugs of abuse during probation period.
22.Must not operate a motor vehicle unless properly licensed,
insured and/or not under suspension.

ANY VIOLATIONS OF RULES, REGULATIONS OR CONDITIONS OF PROBATION
WILL RESULT IN SUSPENDED
DAYS OF JAIL SENTENCE BEING
REIMPOSED.
I have read the above, and the orders of the Court containing
other specific conditions. I understand every rule and the other
conditions of the Court. I promise to abide by them. I realize
that any violation will cause my probation to be revoked and the
suspended sentence reimposed for the offense that I have
committed.

The foregoing rules for all probationers, both General and
Specific, have been approved by the Judge of the Washington C.H.
Municipal Court. A copy of these rules will be furnished to you
and your attorney for reference.
