INFORMATION SHEET FOR PARTICIPATION IN RESEARCH STUDY
EXPLORING COMMUNITY INTER-RELIGIOUS GROUPS
You are being asked to participate in a research study being conducted by Dr. Nathan Todd, Ph.D., at DePaul
University. We are asking you because we are trying to learn more about community inter-religious groups, or
groups where people from different religious congregations meet for a common goal. This study will take about
30-45 minutes of your time. If you agree to participate, you will be asked to fill out a survey including questions
about you and this network. You can choose not to participate. There will be no negative consequences if you
decide not to participate or change your mind later. You need to be 18 years old or older to participate.
HOW TO COMPLETE THE SURVEY
The survey can be completed online at http://nathanrtodd.netfirms.com/religionstudy.html. When asked for
your Group Code, please type in 2105. This code allows us to know which group you are a part of and does
not personally identify you. Knowing which group you are a part of will give us a better picture of your group
so please make sure to include this information. Your responses will not be made available to your group or
anyone besides those on our research team.
The same survey also can be completed on paper and returned to Dr. Todd. If you would like a paper copy of the
survey, please email depaul.religious.study@gmail.com with the name of your group to request a survey. A survey
and postage paid envelope will be provided. We will write your group code on all materials before sending the
survey so that we know which group you are a part of. Please complete the online or paper survey and not both as
they are the same survey.
POSSIBLE DONATION TO CHARITY FOR PARTICIPATION
Having more than one participant from your group will give us a better picture of your group. To encourage
participation, we will make a $15 anonymous donation to a charity if we receive 5 or more participants from your
group. The donation will be made to United Way, World Vision, or the American Red Cross. At the end of the
survey you can designate your choice of charity, and the charity that receives the most votes from your group will
receive the donation. Make sure to include your group code so that we can count your survey toward this donation.
OTHER INFORMATION
Being in this study does not involve any risks other than what you would encounter in daily life. You may skip any
questions you do not feel comfortable answering. You will not personally benefit from being in this study.
However, what we learn will likely help us to better understand groups like yours. When the study is complete, we
also will send your group a report of the findings where we combine responses from all people from all groups who
participated. Your group will not be identified in this report.
