THIS ENDORSEMENT CHANGES THE POLICY. PLEASE READ IT CAREFULLY.

MAJOR MEDICAL EXPENSE COVERAGE

This endorsement modifies insurance provided under the following:

ANIMAL MORTALITY STANDARD COVERAGE FORM

This endorsement is subject otherwise to the exclusions, definitions, and conditions of the policy that it modifies.

This form is subject to a deductible and special limits.

This coverage only applies to those covered animals for which a specific premium charge for Major Medical

Expense Coverage is indicated in the Declarations. The policy is amended as follows:

1. COVERAGE a.

We will indemnify you for major medical expenses that you incur with regard to a covered animal due to an accident, injury or illness which first occurs during the Policy Period. b.

Our obligation to indemnify you for major medical expenses based upon or arising out of any one accident, injury or illness ends:

(1) 120 days from the date of the first treatment; or

(2) 90 days after the expiration of the Policy Period; whichever occurs first. c. Reimbursement for post operative care is limited to the lesser of:

(1) Fifty percent (50%) of the cost of the surgery; or

(2) The actual related costs incurred within the twenty (20) days after the date on which the surgery is performed. d.

Reimbursement for treatment arising out of a definitive diagnosis of gastric ulcers is limited to sixty (60) days from the date of the first treatment.

2. EXCLUSIONS

This policy will not apply to claims arising out of, resulting from, directly or indirectly caused by, or as a consequence of: a.

Any accident or injury that occurred, or sickness, illness, disease or medical conditions existing, diagnosed, or treated prior to the effective date of this endorsement; b.

Veterinary travel expenses, animal transportation fees or veterinary call charges; c.

Services performed by anyone other than a veterinarian , unless prescribed by a veterinarian ; d.

Surgery performed at any premise other than a: e.

f.

(1) Veterinary surgical clinic; or

(2)

SPECIMEN g.

Any treatment associated with the maintenance of an animal for its intended use, including compliance with breed standards;

h. Any treatment, supply, device, or level of service if it is for custodial care, or is experimental, investigational, or educational in nature; i.

Unfocused extracorporeal shock wave therapy (also known as radial pressure wave therapy or RPWT).

Refer to the Special Limits within the Limit of Insurance section for additional limitations; j.

Any elective surgery and/or procedure including, but not limited to: castration, Caslick’s operation, cosmetic surgery, neurectomy, surgery to correct laryngeal hemiplegia or entrapment of epiglottis; k.

Any alternative medical treatments or procedures, including but not limited to: chiropractic treatments, massage therapy, acupuncture, mesotherapy, magnet therapy, whirlpool, aqua-tread, treadmill treatment, hyperbaric chamber, laser treatment, dietary supplements, megadose vitamins, herbal preparations and/or other alternative, homeopathic, performance enhancing medication , treatments or procedures; l.

Intra-muscular, intra-articular or intra-venous synovial fluid replacement therapy or viscosupplementation, or any method of administration of hyaluronic acid, hyaluronate sodium, corticosteroids, polysulfated glycosaminoglycan, etc. whether or not used in conjunction with any other treatment method or used separately; m.

Any surgical and/or medical treatment or diagnostic testing for or relating in a manner to any flexural or angular limb deformity (regardless of its cause or origin, and whether or not evident at the effective date of this endorsement), including but not limited to club foot, valgus or varus deformities, bucked shins, knock knees, and contracted digital flexor tendons; n.

Any treatment of a congenital birth defect, including but not limited to, acquired or developmental defects, growth or nutritional conditions, umbilical or scrotal hernia, cryptorchidism, hyperkalemic periodic paralysis (HYPP), hereditary equine regional dermal asthenia (HERDA), severe combined immune deficiency syndrome, or contracted tendons, whether evident or not at the effective date of coverage for the covered animal ; o.

Any surgical and/or medical treatment or diagnostic testing for or relating in any manner to any complication arising from the treatment of any accident, sickness, disease, injury, lameness, illness, or physical disability otherwise excluded from this additional coverage; p. Corrective or therapeutic shoeing, including related consultation expense; q.

Any dental treatment for a non-life threatening dental condition unless necessitated by visible external trauma; r.

Treatment due to any malicious or willful injury or poisoning or gross negligence caused by you; s.

Any animal used for racing, race training or intended for racing. This does not include competition and stock horses while participating in timed events or classes; t.

Autopsy or post mortem procedures; or u. Carcass disposal and any other disposal charges.

3. LIMIT OF INSURANCE

Coverage.

Our obligation to indemnify you for major medical expenses of any covered animal will be in excess of the deductible, up to, but not exceeding the applicable Limit of Insurance for Major Medical Expense

Coverage shown in the Declarations.

Special Limits a.

The most we will pay for each separate incident of lameness that does not require surgery to resolve is fifteen (15) percent of the available limit of insurance for Major Medical Expense Coverage. b. We will pay expenses for diagnostic radiographs and ultrasounds. We will pay eighty percent (80%) of the expenses for all other types of diagnostic medical imaging procedures. c.

Gastric ulcer medication given as a preventive measure in conjunction with other covered treatments is limited to $120 per incident. d.

The most we will pay for focused extracorporeal shock wave therapy is $1,200 per covered animal for each separate, unrelated, and nonrecurring incident of accident, injury, or lameness, whether used exclusively or in conjunction with other treatments or therapies.

4. DEDUCTIBLE FOR MAJOR MEDICAL EXPENSE COVERAGE

The coverage provided by this endorsement is subject to a deductible of $275 for each separate claim during the Policy Period. In the event of any recurring accident, injury or illness, the deductible will apply separately to each such accident, injury or illness, including any related surgery.

5. DEFINITION

Major Medical Expense means fees or charges by a veterinarian for treatment or surgery, including postoperative care, which is required or necessary as a result of an accident, injury, or illness.

6. PROOF OF CLAIM

You must give us immediate notice of any accident, illness, or injury that requires or necessitates medical care and treatment, including any surgical procedure, to a covered animal . For the purposes of this endorsement only, you must: a.

Forward to us a report signed by the treating veterinarian describing the care and treatment performed and describing the covered animal’s condition immediately before and after such treatment; and b.

Furnish us with copies of service bills or invoices showing all Major Medical Expense for which the claim is being made within sixty (60) days after the care or treatment concludes.

7. FULLY EARNED PREMIUM

If you cancel this coverage or the policy to which it is attached, we will retain the entire premium charged for this Major Medical Expense Coverage endorsement, as the premium is fully earned upon issuance. If we cancel, we will issue a pro rata refund of any unearned premium for this Major Medical Expense Coverage endorsement.

All other terms and conditions of the policy remain unchanged. SPECIMEN
