any public forum through any medium unless expressly required by state or federal law. Any other use or disclosure of your SSN will
require your written authorization.
Definition of Terms Used in this Enrollment Agreement / Contract for Training
 Supply / Lab Fee is inclusive of all products, materials, and supplies available to and utilized by the student upon
themselves, classmates and models during pre-clinic training.
 Period of Enrollment is the time elapsed between the student’s actual start date, and the date of the student’s last day of
physical attendance at Beau Monde Academy of Cosmetology.
 Retail Installment Agreement and Cash Payment Schedule are inclusive and a part of this Enrollment
Agreement/Contract for Training.
 All students enrolled at Beau Monde Academy of Cosmetology are considered full time students. Both 35 and 24-hour
students are considered full time. 24-hour students are considered full time students on a part time schedule.
Course and Training Objectives
The training program in which the student is enrolling is intended to prepare the student for the Health Licensing Office Examination,
licensure, and subsequent employment in the Full Course Cosmetology, Hair Design, Barbering, Nail Technology, Esthetics, or a
related professional field.
Additional Fees that May be Incurred
 In signing this Enrollment Agreement/Contract for Training, the buyer recognizes and agrees to pay all additional fees
that may be assessed to the student account, based upon the following particulars:

Course/Training Program Transfer Fee – a student enrolled at Beau Monde Academy of Cosmetology may transfer to
another program by requesting an IN SCHOOL TRANSFER in writing. Students are required to provide notarized
documentation and the request must be approved by the CEO. The student will be required to execute a new Enrollment
Agreement/Contract for Training and will be charged an administrative transfer fee in the amount of $100.00, at the time
of each course transfer. The sole exception to this Policy is applicable only to a student who desires to transfer before
five days have elapsed from their contractual start date. A student wishing to transfer their course of training is herein
advised that in some instances, not all competencies will be transferable.
Schedule Change Fee- a student may request a written schedule change. A schedule change fee will be charged in the
amount of $25.00 at the time of each schedule change. A student wishing to change their schedule is herein advised that
each Schedule Change directly effects and changes the estimated date of completion, listed in an addendum to the
contract.
Overtime Charge – Students will incur overtime charges from the point the student completes the State of Oregon clock
hour equivalent of their course at the rate of $240 per week.
Withdrawal Fee and Withdrawal Requirement – a student seeking to withdraw from training, or otherwise terminate this
agreement, must complete, sign and date a WRITTEN “Notice of Withdrawal” to Beau Monde Academy of
Cosmetology. Failure, on the part of the student to provide a Notice of Withdrawal will result in the assignment of an
official withdrawal date after thirty consecutive days of non-attendance. A withdrawal Fee in the amount of $25.00 is
assessed to the account of all withdrawing students. Within 40 days of the last day of attendance, a refund calculation is
performed.
Returned Check Fee – should a student’s personal check be returned as non-payable, for any reason, a Return Check Fee
in the amount of $35.00 is assessed to the student account. The student will be required to redeem the bad check either
with cash or a money order within 10 days of the academy’s receipt of the Returned Check.
Late Payment Fee – Payments are due and payable the 1st day of the month. Payments received after the 5th day of the
month will be charged a $25 fee. Should a student fail to respond by rendering payment due to the school or by way of
written explanation and rescheduling payments, the student may be automatically terminated from the school for nonpayment of the institutional charges. Unpaid account charges will be turned over to a collection agency. Reasonable
attorney charges, collection, and court fees and costs will be awarded to the prevailing party in any action undertaken to
collect upon the student’s contact or any part of it.
Saturday Attendance Requirements – Students must attend classes on their scheduled Saturdays. Student must clock a
minimum of 3 hours of attendance per day to be counted as attending on a Saturday. Consequences for missing Saturday
sessions are $25 per Saturday missed after the first three. More than 10 Saturdays missed and the student may be
terminated.
Enrollment and Admissions Requirements (for full disclosure see Catalog/Handbook)
All students seeking admission to Beau Monde Academy of Cosmetology must satisfy the following admission requirements, the
applicant must:
 Fully complete and submit the “Student Inquiry” form
 Be beyond the age of compulsory education in the State of Oregon, being no less than 16.5 years of age
 Must provide proof of identity and age in the form of their Social Security Card, and a current State issued Driver’s
License or State issued Identity Card, for retention in their permanent record. Please refer to page 12 in the catalog.
 Must pay the Registration Fee of $150.00 plus 50% of the total sum of all institution charges, or make satisfactory
payment arrangements for the remaining balance of the institutional charges due to the academy through completion of
the Retail Installment portion of this Enrollment Agreement/Contract for training.
 Students are required to provide a High School diploma, GED, or equivalent.
 If Agency sponsored, the applicant must provide a letter or voucher from the sponsoring agency to the academy.
Transfer or Re-Starting Student
Beau Monde Academy does not typically accept credit for prior training and reserves the right to review requests for transfer of credit.
If the request is approved a student previously enrolled at another postsecondary cosmetology institution, or seeking to re-start their
training course, who desires credit for previously earned competencies, must request and present an Official Academic Transcript to
the academy, prior to enrollment. The determination of previously earned competencies accepted for institutional training credit and
institutional charge adjustments, is made solely at the discretion of Beau Monde Academy of Cosmetology. Previously earned
competencies earned by the student that are 10 years, or older, shall NOT be considered for transfer or re-starting student credit.
Previously earned out-of-state competencies for “Health Safety and Sanitation”, shall NOT be considered for transfer, as these
competencies VARY from state to state, and do not reflect current Oregon statutes or Administrative Rules.
Transferability of Competencies Earned at Beau Monde Academy of Cosmetology
Beau Monde Academy of Cosmetology makes no guarantee of the transferability of competency credits earned at this institution, to
another postsecondary institution.

Individual Proficiency Based Prescriptive Training Course/Program
Courses are taught in a series of sequential training steps. The student must pass all theory and practical tests administered throughout
their training in order to demonstrate mastery and advance in their course of training. Courses are divided into Pre-Clinic, Clinic Level
1, Clinic Level 2 and Salon Ready levels. Students are allowed to complete each level as quickly as the student demonstrates mastery.
Course tuition charges are structured to include all academy theory and practical units. Beau Monde Academy of Cosmetology has
determined the cost of each unit.
A student’s Proficiency Based Prescriptive Training Course will NOT be significantly altered in any manner, once the student and
Beau Monde Academy of Cosmetology have signed this Enrollment Agreement/Contract for Training.
Any revisions to Beau Monde Academy of Cosmetology’s Prescriptive Training Course are based on the result of recorded student
performance evaluations and student negotiations. A copy of each revision to a student’s Prescriptive Training Program is made in
writing and a copy of each revision provided to the student.

Conditions of Continuing Enrollment
In order to create and maintain a healthy, safe and professional work and learning environment for all students, staff and clients, and
as a condition of continuing enrollment, the student in signing this Enrollment Agreement/Contract for Training agrees to abide by and
comply with all institutional Rules, Policies and Procedures. Failure on the student’s part, to abide by and comply with all institutional
Rules, Policies and Procedures, inclusive of failure to maintain all published Satisfactory Progress Standards, may result in permanent
separation from the academy.
*Satisfactory Progress Policy – includes two cumulative elements of measurement. As it regards attendance, the student is required
to maintain a minimum cumulative attendance percentage of no less than 70% of all scheduled training time. As it regards Academic/
Practical Operations/Requirements the student is required to maintain a minimum cumulative grade percentage of no less than 75% of
all requirements.
*Unsatisfactory Progress – a student terminated by the academy for unsatisfactory progress will not be considered eligible to apply
for re-enrollment until a period of no less than 60-days has elapsed from the termination date. A student applying for re-enrollment
will be required to execute a new Enrollment Agreement/Contract for Training. Beau Monde Academy of Cosmetology reserves the
right to deny the right of re-entry to any student previously terminated for unsatisfactory progress or any other contract violation.
*Immediate Termination – any student who enters institutional property while in possession of; or while under the influence of
alcohol, illegal drugs, narcotics, or un-prescribed controlled substances; or while in the possession of any type of weapon or fireworks
will be immediately terminated with cause.
*Hostile Environment – any student violating the academy’s Hostile Environment Policy through aggressive or threatening acts
towards another will be immediately terminated with cause.
*5 to 7 Salon Services – Beau Monde Academy requires all students on the salon floor to perform 5 to 7 salon services daily. In lieu
of not receiving 5 to 7 clients a student may perform services on a mannequin.
*Applied Effort Policy – see catalog for details.
Graduation Requirements
In order to graduate the course of training each
student must:
 Complete all competencies listed on their student catalog/handbook for training, and maintain a cumulative attendance
percentage of no less than 70%.
 Take all required theory (academic) tests and maintain a cumulative test average of no less than 75%.
 Take all required practical (hands on) tests and maintain a cumulative test average of no less than 75%.
 Complete all practical requirements and demonstrate mastery in practical operations.
 Take and pass the institutionally administered theory and practical final examinations (referred to as the “Baby Boards”),
achieving a grade of no less than 75%.
 Complete all employment seeking forms.
 Complete all exit interviews
 Have no outstanding financial obligations to the academy. The student account must be paid in full.
A student who satisfies the requirements listed above is considered a graduate and is issued their diploma, upon request. Within 7 to
10 calendar days the academy will release the student’s official academic transcripts directly to the HLO.
No Reduction of Institutional Charges - No reduction of institutional charges is considered should the student complete their
contractual course of training prior to the estimated or projected completion date.
Academic Credential - At the time that a student successfully completes requirements for graduation from their course of training a
signed and dated Diploma is issued by Beau Monde Academy of Cosmetology, upon request.
Employment and Placement - Beau Monde Academy of Cosmetology makes no guarantee of future employment compensation to
any student. Employment and placement assistance is provided by referral only.
Institutional Disclaimers:
 *The academy will not be held accountable or responsible for any agreements that are not in writing and not a part
of this contract.
 *Hazardous chemicals and solutions are used and maintained by the academy and are routinely used for professional
applications. Students are instructed to immediately notify a staff member should an accident or a physical reaction
to a product occur. Based upon this disclosure and disclaimer this institution will not be held liable for any incidents
regarding chemical exposure, accidents, or reactions.

*The academy will not be held responsible for any personal articles lost, damaged, or stolen from its premises.
Further, the academy will not be held liable or responsible for personal vehicle or the contents of the vehicle while
utilizing the parking lots or public parking area.
 *No responsibility is assumed by the academy for any negligence, carelessness, or lack of skill by one or more
students while practicing any part of the practical curriculum requirements upon one another.
 *The academy makes no guarantee that all or part of the practical operations will be performed upon paying clients.
 *Beau Monde Academy reserves the right to search student belongings and lockers if they are not in compliance
with all OSHA safety sanitation and regulatory agencies rules.
Student Review Grievance and Complaint Policy - A student having questions regarding this Enrollment Agreement/Contract for
Training or aggrieved by the actions of this institution should first try to resolve these issues with the appropriate institutional officials.
Should this procedure fail, the student may file an internal complaint. See page 44 in the academy catalog for the internal complaint
procedure. Should this procedure fail, the student may contact in writing, the Higher Education Coordinating Commission at 775
Court St. NE Salem, OR 97301 or NACCAS at 4401 Ford Avenue, Suite 1300 Alexandria, VA 22302. This policy applies to any fee
disputes that may occur.
Refund Policy - Buyers Right to Cancel – a student may cancel their enrollment by giving written notice to the academy. If the notice
occurs within 5 days of the date of enrollment, all monies paid shall be refunded; or if cancellation occurs after 5 days of the
enrollment, but prior to starting training, the academy is entitled to the $150.00 non-refundable registration fee.
THE STATE OF OREGON – 50% PRO-RATA: SCHEDULED DAYS UNTIL THE STUDENT’S LAST DAY OF
RECORDED ATTENDANCE – 715-045-0036
A student’s period of enrollment is defined as the period of time that begins on the student’s first day of training (start date), and
accrues until the student’s last day of attendance at the school.
If a student withdraws prior to completion of 50% of the contracted program, the student shall be entitled to a prorate refund of the
tuition charged and paid for such instructional program, less registrations fees, and any other legitimate charges owed by the student.
If a student withdraws upon completion of 50% or more, of their scheduled and contracted instructional program, the student shall be
obligated for the entire instructional program, and shall not be considered entitled to a refund.
The term pro-rata refund means a refund of tuition paid for that portion of the program beyond the last recorded date of attendance. In
calculating a pro-rata refund, the proficiency based program’s measuring the period of enrollment in scheduled training days; the
amount the student will be charged is determined by dividing the total number scheduled days accrued by the student according to the
published class schedule as of the last day of attendance by the student.
The published class schedule is equal to the number of scheduled training days the academy has made available and provided to the
student, had the student maintained 100% attendance.
The number of days that the student was scheduled to complete is divided by the number days scheduled in order for the student to
complete the course/program. The result is the percentage of the course/program that the student completed. The resulting percentage
is used to determine the student’s tuition debt to the academy.
The maximum number of scheduled training hours in each respective course is:
 Safety, Sanitation, and Hygiene – 150 hours
 Career Development – 100 hours
 Hair Design – 1450 hours
 Nail Technology – 350 hours
 Esthetics – 250 hours
 Full Course Cosmetology – 2050 hours
 Hair Design and Nail Technology – 1800 hours
 Hair Design and Esthetics – 1700 hours
 Nail Technology and Esthetics – 600 hours
 Barbering – 1100 hours
 Cadet – 1000 hours
A withdrawal fee in the amount of $25 is charged to the account of a withdrawing student.
United States Department of Education ~ Return of Federal Student Financial Assistance Program Funds
The United States Department of Education requires an additional calculation to be made for a student who has received Federal Title
IV Assistance for the period of enrollment. If the hours actually completed by the student are less than 60% of the payment period, or
total course hours, a portion of the assistance awarded to the student is considered unearned and must be returned to the appropriate
Title IV program. This may create a larger than expected gap between the amount the school may charge the student and the amount
of financial aid available to pay institutional charges. The student is liable for payment to the school of charges earned but not
provided for under this required formula. Title IV funds, such as Federal Pell Grants that are considered unearned by the student must
be returned to the appropriate Federal Program

Assignment of Last Day of Attendance, Return of Monies if Due, and Dismissal or Termination of Student:
In the instance that the student withdraws or is terminated from training by the academy, all monies due or otherwise overpaid to the
students account are refunded to the appropriate agency or person within 40 days of formal withdrawal, cancellation, or termination.
The student is notified in writing of the account status, all institutional charges, and refund made, if applicable.
In lieu of a formal written withdrawal not from the student, formal termination shall occur no more than 40 days after the student’s last
recorded date of attendance at the academy. In the instance of a student who does not return from an institutionally approved Leave of
Absence, the student’s documented anticipation date of return from leave will be used as the termination date.
A student who is expelled or otherwise terminated from Beau Monde Academy of Cosmetology will not be entitled to a refund other
than as specified and published herein.
Contractual Disclosures and Assertions
The buyer agrees, by their signature, they have received a fully completed copy of this Enrollment Agreement/Contract, a
copy of the Institutional Catalog and Required Student Disclosures.
The buyer asserts that NO GUARANTEES of employment or starting wage scales have been made, either in writing or
verbally.
The buyer asserts that they have fully read all pages of this Contract and the Institution has answered any questions that the
Buyer may have. The Buyer further understands that they are free to take this Contract to any outside third party for review and
explanation, prior to signing.
The buyer asserts that prior to signing this Contract; they have been made aware of the legal obligations for which they are
assuming responsibility, by entering into this contract for training.
The buyer understands that unpaid account charges will be turned over to a Collection Agency. The buyer agrees that
reasonable attorney, collection, and court fees and costs will be awarded to the prevailing party in any action undertaken to collect
upon this Contract, or any part of it.
Fees for extra/ additional items such as personal items used by the student for personal services, or requested services for self,
need not be considered a part of this contract, or in the refund calculation, provided these fees have been itemized and separately
charged to the student. The process for purchase of extra consumable supplies is located at the Reception Desk.
The buyer understands that this Contract becomes a legally binding instrument only when accepted and signed by the student
(Buyer), their parents, if a minor, and a duly authorized representative of Beau Monde Academy of Cosmetology.
The buyer and their co-signer, if applicable attest by their signature and assert that all statements made herein are true and
accurate to the best of their knowledge.
Upon the execution of this contract by all applicable parties, the student is herein protected from any and all increase of charges made
by this institution at a later date. The exception being that of a student who has dropped, withdrawn, or otherwise terminated this
contract; thereby severing the institutional charges associated with this Enrollment Agree/Contract.
Notice To Signer & Co-Signer of Debt (FTC Notice) in all Consumer Transactions the Federal Trade Commission requires that
written notice be given to the person who is co-signing an agreement which they will have to pay if the primary debtor does not. As a
co-signer you are being asked to guarantee all charges associated with this Contract. You are entering into a serious and binding
Agreement. If the student does not pay the debt, you will have to. You may also have to pay late fees, collection, and attorney and or
court costs, which increase the amount of the Contract. The academy can collect this debt from you without first trying to collect from
the student. The academy can use the same collection methods against you that can be used against the student. Be certain you can pay
this debt.
Signatures and Dates Time is of the essence for this Enrollment Agreement/Contract and all terms designations contained herein
shall be deemed to have gender, number and entity applicable to all parties who execute this legal agreements, This Enrollment
Agreement/Contract for Training is (7) pages in length, all are a part of this Agreement/Contract and one is not valid without the other
when presented and signed by all parties. In signing this Enrollment Agreement/Contract the student acknowledges that they agree to
pay Beau Monde Academy of Cosmetology the sum of $____________ for the educational and training services described herein;
which will be provided to the student named herein. Beau Monde Academy of Cosmetology in fulfilling its part of the Enrollment
Agreement/Contract for Training herein admits the Enrollee identified in this document as a STUDENT, at its training facility and
herby agrees to furnish said student with their contracted program of training, as stated and described within the context of the
Enrollment Agreement/Contract for Training.
