Terms & Conditions
1. Course registration is on "First Come, First Served" basis. Full payment must be received 10 days prior to course commencement. (Accomodation is not included in course fees)
2. WQSSB reserves the right to cancel or postpone a course 7 days prior to course commencement if attendance is insufficient. In the event of cancellation, WQSSB will offer a full refund.
3. If a registered course is rescheduled to another date, WQSSB will retain the course fees for one calendar year from date of course cancellation. The candidate will be informed of the new
course date.
4. If a registered candidate wishes to cancel, a written request must be submitted 10 days prior to course commencement. A cancellation fee of MYR300.00 will be imposed. No
refund will be given if a cancellation request is received 5 days or less prior to course commencement, or there is “no show” by the registered candidate. Exceptions will be made for emergency
medical conditions whereupon course fees will be credited to the next course.
5. All cheques, bank drafts/money orders, wire/money transfers are to be made to WQSSB.

INTRODUCTION
Over the last few years, legislators have been focusing
on Process Safety as a method to reduce the risks posed
by hazardous industries. This has led many companies
to appreciate the benefits of formal risk assessment
methods as the starting point in the risk management
process. HAZID and HAZOP is recognized as being a
critical technique in the implementation of a successful
risk management system.

ABOUT THE INSTRUCTOR
Mr. Chandratre is the Deputy General Manager for Mumbai office with Chilworth from
last 4 years. He is a Chartered Engineer (Chemical) with Post Graduate Diploma in
Industrial Safety. He has completed a course on Hazards Analysis in Chemical Industries
from Indian Institute Of Technology, Mumbai. He is the Lead Auditor for ISO 9001:2000 and
having 25+ years of experience in Process Safety, Operations,Designs & HSE. He
holds Accreditation Certificate from State Government Authorities as Safety Auditor.
He is a versatile HSE (Health, Safety & Environment) all-rounder as well as a risk specialist, and an accomplished communicator. He has functioned in consultant, managerial, technical, operations support and training
roles and has extensive experience in leading HAZOP (Hazard and Operability) and HAZID (Hazard Identification) studies, process safety improvement, risk assessments of all kinds, technical safety studies, workforce
training, Safety Case development and liason with International Regulatory Bodies. He has the range and
depth of experience to help make major commercial and operational decisions and deliver against corporate
policy on safety, environmental and sustainable development issues.
