New Jersey 2015 Sepsis Learning – Action Collaborative October 2015 CURRENT COLLABORATIVE NEWS SPOTLIGHT ON SEPSIS Improving care of sepsis patients in the post-acute care setting has been the focus of a collaborative pilot group since earlier this year. The pilot group consists of six teams, including three rehabilitation hospitals, HealthSouth Rehabilitation Hospital of Toms River, St. Lawrence Rehabilitation Center and HealthSouth Rehabilitation Hospital of Tinton Falls. The September webinar highlighted the efforts of those three rehab teams and the importance of early identification and management of sepsis in the post-acute care setting. The teams described how they are partnering with their acute care hospital partners, engaging multidisciplinary staff in processes for early recognition and treatment of sepsis, and measuring acute care readmissions related to infection and sepsis. The pilot teams helped to develop the NJ Toolkit for Post-Acute Care Settings which is available for free at www.NJHA.com/sepsis.
