How to make an appointment: Contact Phlebotomy Booking Line between 9am and 5pm Monday to Friday 020 8630 3060 } call queuing system 020 8630 3058 } Have your test form with you - the receptionist will ask for your NHS number and test details recorded on the form. Or email requests to phlebotomy.hrch@nhs.net, stating name, NHS number and contact details. Central blood test bookings will aim to contact you within one working day to book an appointment. The Phlebotomy clinics are based at Heart of Hounslow Centre for Health, 92 Bath Rd Hounslow and Chiswick Health Centre. We operate a variety of clinic times at each location – Mon – Fri. Arriving for your appointment: Allow time for parking at the Health Centre. Please arrive and report to clinic reception at least 10 mins prior to your appointment with your blood test form. Arriving more than 10 minutes late in reception may result in your appointment being allocated to another patient or you having to re-book. Please drink 2 glasses of water at least 2 hours before your appointment; this makes it easier to take a blood sample. Consent: In order to take the blood test we do require consent from the patient. We are unable to carry out the blood test without this consent. If you have been asked to fast for Glucose and/or Cholesterol/Lipids blood tests: Please make a morning appointment for the test. Make sure you have nothing to eat or drink for 12½ hours before your blood test. You are only able to drink water. Please ensure you have drunk at least two glasses of water prior to your appointment. Issued: May 2012 Written by: Teresa Keegal Jan Nimmo Review date: April 2013 Information for patients Blood test booking made easy: ADULTS Glucose loading/glucose tolerance tests (GLT/GTT): If your blood test is for a GLT/GTT inform the Phlebotomy Booking Line you will need 2 appointments 2 hours apart. You should have nothing to eat and only water to drink for 12½ hours prior to your appointment. Please drink as much water as you need, but at least 2-3 glasses in the morning. You must bring a 380/500ml bottle of Original Lucozade with you to your appointment and drink the whole bottle immediately after the first appointment. Please do not eat or drink anything else until after the second appointment.
