NOTE: Students receiving internship credit MUST enroll in ITS 4910, Internship, during the
semester in which the internship is taken. The amount of credit taken should be discussed with
the Internship Coordinator and/or your academic advisor.

internship, I authorize the School to enroll me in ITS 4910 during the semester that the internship
takes place for up to
