www.lellikelly.it , of the best pictures of children wearing the flashing braid in their hair. To ensure that the pictures sent may be used
for this initiative and, if successfully selected, be published on our website, it is necessary the authorization of both the parents of the
minor portrayed. Hence we invite You to carefully read the following Information and Authorization, to fill the empty lines with the
missing data, to print the page, to sign it, and finally to send it by mail to LELLI KELLY S.p.A. – Via Bocchi 233 – 55013 LAMMARI
(Lucca) - ITALY.
Should You not give us the required authorization, we will not use in any way the pictures that we received from You and we will
delete the file.
Information and authorization to process personal data under the Legislative Decree n. 196/2003. Permission to publish.
Under sections 7 and 13 of Legislative Decree n. 196/2003 (hereinafter “D.Lgs. 196/2003”) Lelli Kelly S.p.A. provides this information

Personal data of the People concerned will be fairly processed by Lelli Kelly, in compliance with the most up-to-date security
standards, either using electronic devices and computer software or not. The data processing is directed exclusively to Lelli Kelly’s
initiative, which consists of selecting pictures of children (girls only) wearing Lelli Kelly’s flashing braid, and publishing the best ones
(selected among them with final decision by Lelli Kelly S.p.a.) on the website www.lellikelly.it .
Data of the People concerned will be processed carrying out the operations or set of operations listed in section 4, paragraph 1, lett.
a), of D.Lgs. 196/2003, i.e.: collection, recording, organisation, keeping, interrogation, elaboration, modification, selection, retrieval,
comparison, utilization, interconnection, blocking, communication, dissemination, erasure and destruction of data.
The People concerned are aware that their authorization to the processing of the data mentioned above is a necessary condition for
Lelli Kelly S.p.A. to use the pictures received, in the ways and for the purposes mentioned above. The People concerned are aware
that under section 7 D.Lgs. 196/2003 they have the right to: access their own personal data; request for their personal data to be
updated, rectified and erased, when incomplete, false, or obtained in violation of the law; object, on legitimate grounds, to the
processing of their own personal data. The rights referred to above may be exercised by making a request to the data controller.
The People concerned may address their requests to the data controller, Lelli Kelly S.p.A., either by writing to its site in 55013
LAMMARI (Lucca), Via Bocchi n.233, ITALY, or by faxing to the n. 583962756, or by e-mailing at info@lellikelly.it.
By signing this information, the undersigned express their assent to the processing of the personal data they issued, both by filling
out this form and by sending the pictures of their underage daughter, within the limits and for the purposes mentioned above.
The undersigned declare that they made the pictures sent to the website www.lellikelly.it and that, in any case, they have the right to
publish them or have them published. By signing this information, the People concerned authorize the publication of the pictures free
and indefinitely on the website www.lellikelly.it.
