Introductions

 Members briefly introduced themselves and their agency affiliations

Review and endorse minutes from last council meeting

 Members reviewed the minutes dated 10/7/08. The minutes were approved unanimously.

Update from ITOP Coordinator –J. Weber

 John briefly reported on this month activities. He provided a preview of the ITOP Logo final draft.

He reported October has been a very busy month for the Council filled with activities.. He stated he is diligently implementing the activities of the 2008-2009 ITOP Work Plan. He appreciates and values everyone’s times, support contributions and commitment to our common goals as an interagency council.

Standing Committee Report: HRC, Mayor’s Transitional Youth Task Force

 TAY has had to reschedule November meeting. The official date has not been posted. The TAY is seeking to develop a Providers Network to engage in planning and discussion with the TAY

Transition Improvements in San Francisco.

Youth Council Report – Daisy /Ben

 Daisy reported on the activities of the Youth Council. Youth Council members were encouraged to participate in the historic Presidential elections. They were encouraged to help get out the vote and vote themselves. Daisy also mentioned the outreach and presentation at the High School Transition

Fairs. She also said for the new youth Council applicants, we hope to do a new member orientation in January. Also, because of lack of funding the ITOP Council will need to postpone the health walk until further noticed. She reported that the next ITOP meeting we hope to have a hotdog and or

DVD Movie night on the 3 rd Floor patio. The next ITOP meeting is December 3 rd , 2008.

Guest Presentation: San Jose Social Recreation Research Project with Transition Aged Youth

 Dr. Gonzaga reported on his research project on Social and Recreation, Leisure in the Transition

Aged Youth. He hopes to utilize the ITOP interagency council and the ITOP Youth council as an expert panel to review the finding of his research Project. Members engaged in a brief discussion about the research project details. Dr. Gonzaga agreed to keep the Council informed on his progress.

INFORMATION AND TRAINING ne

Transition Dream Team Update (On-site High School Resource Fair) Robin, Linda, Jack

 Judy reported that the groups were scheduled to complete 3 transition resource fairs, Ida Wells and

Washington High Schools and Lincoln High school. The next transition dream team meeting is scheduled for November 13th at 3pm. The meeting will be held at the Northern California Hearing and Speech

Center on Divisadero. Lincoln Transition Resource Fair is scheduled for December 9 th , 2008..

Transition Track for SFCD I&R Conference, Linda T / Jacy C

 Members met last week to decide which topic will be presented at the SFCD Information and Resource

Conference. The presenters need to confirm their participation by 11/12/08. Rocio from Area Board V was invited to present on” My Youth Is Turning 18…. What Now? The information and Resource th Conference will take place on March 28 , 2008. Please remember to save the date on your calendar. Jane requested that we place this information on the all district school “robo” call. Linda Tung, Jane Steiner agreed to follow up with this approach.

II. SUPPORT & OPPORTUNITIES

Adopt – A- School Project in 3 Schools – Jack/Robin/ Judy

 Judy Doane provided a background as to how the Transition Dram Team was developed. She also linked the historical context of how The Adopt – A – School was created. The original 3 schools were Ida B. Well,

Washington and Lincoln High School. She informed the group on the improvements and impact of Adopt

A School with the increased enrollment rate of Transition Aged Youth with Disabilities in City College.

Members engaged in discussions about the Adopt- A-School and about options those students who may not attend the City College. Members aslo asked about the follow-up with youth as they make progress in transitioning towards their Jr. College experience. Jacy suggested that we develop Career search function on our website for youth with specific needs. The career search tool/ application can be placed on the product section of the Website. Juno also suggested that youth Council develop group discussion on how we motivate youth. Daisy agreed to bring this to the Youth Council.

ITOP Youth Council – increase membership to 20 youth – Daisy

 There were approximately 49 youth contacts that were made on the sign-in Sheet at Washington Transition

11-12-08 Draft COUNCIL Minutes fair. The ITOP Group is has approximately 16 members to- date.

Youth Led IEP Project-

 Ben Reported that The IEP Youth Group continues to meet. The group is editing and modifying 3 documents. Support For Families IEP Training Guide, NICHCY’s Helping Youths Develop Their IEP’s the Student Guide to the IEP. The IEP team will take the best elements of all three documents. Finally, the group is in discussion as to who else should be invited to the table.

Quarterly meeting with the Special Education, SELPA Director /Administrator of Secondary – J. Weber

 The Quarterly meeting with the Special Education SELPA Director, David Wax, Administrator of

Secondary Special Educations Linda Ellis is scheduled for November 21, 2008

OUTREACH

Presentations related to transition at 3 citywide agencies – J. Weber & Daisy

 The scheduled November 13 th presentation was postponed by DOR until December 11

Office on Disability is targeted for January. th . The Mayor

Open House Arc- Jacy C.

 The ARC Open House took place on November 10 th . There were over youth 75 attendees who signed in, 25 parents and 15 professionals and 24 youth self identified on the sign-in sheet.

Lifehouse, Department of Rehabilitation, Golden Gate Regional Center, Support for Families,

ITOP, Job Corps, and Opportunities for Independence Marin Autism Collaborative were among the community base organization that attended. Approximately 10 schools participated in the open house. The participating school included, Mission High, SOTA Lincoln High, Lowell High,

Wallenberg High, Thurdgood Marshall High, John O’Connell High, Jefferson Union High and

Redwood High. Jacy also thanked Daisy, ITOP Youth Council, ITOP Interagency Council and

John Weber for their involvement and assistance. She, Alan and Mr. Hornbecker were very glad and appreciative. Overall, Jacy reported that the Open House was a great success.

IV.

SUSTAINING

Securing of funding for ITOP Council’s – J. Weber

 A draft outline for grants has been developed by the Steering Committee. ITOP with the assistance of Jan Watson, the development director is applying for Wells Fargo and United Way Funding

Opportunities/ Grant..

Document detailing accomplishments since inception of ITOP- . J. Weber

 The draft of the top 10 ITOP accomplishments is still being edited. Once the document is completed, we will send a copy to the Council for final review.

THE FOUND GENERATION – JACY C. V.

 The final proposal was submitted to DCYF. Alan Fox improved the proposal for re-submission.

The ARC and Found Generation Team is making steady progress. Unfortunately, DCYF due to budget cuts, the Found Generation Grant is on hold. We will keep the Council updated on the progress. The Found Generation Team is scheduled to meet later this month.
