Welcome to Minster Medical Practice
The doctors and staff at Minster Medical Practice are proud to offer the highest standard of patient-centred
healthcare. We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a
wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations
and well-person check-ups.
In addition to everything you need to know about the practice you will also find a wealth of health-related
information in the menu on the right hand side.
ORIGINS
Historically our Practice has been in the Cathedral Area since the mid-nineteenth century. Drs Maclure
and Watson moved from Eastgate to "2 Mainwaring Road" in 1968 and the Practice remained there until
1991, when we moved to our current location.
NOW
Our Surgery is situated off Nettleham Road in the Cabourne Court Health Care Complex. This site has
ample car parking and pedestrian access from both Nettleham Road and Cabourne Avenue. Other
services on the site include a Chemist, Dental Practice, Physiotherapy clinic, Chiropractitioner,
Chiropodist and Hearing Care Aid Centre.
GENERAL PRACTITIONER REGISTRARS
We have been a training Practice since 1994. Dr Pontin and Dr Fitzgerald are the approved trainers. GP
Registrars join us for four or eight month attachments. They are fully qualified Doctors, but have not yet
completed the specialised extra training now required before they can become a partner in General
Practice.
Because we undertake training of other Doctors wishing to enter General Practice we undergo a periodic
assessment when the quality of the records in the Practice may be examined by senior Doctors.
Patient confidentiality is always maintained.
However, if you do not want your notes to be examined please inform the Receptionist or your Doctor.

When We Are Closed
During the week from 6.30 pm to 8.00 the following morning, at weekends and Bank Holidays when the surgery is
closed please ring 111. This is a freephone number from both landlines and mobile telephones. This allows you
direct access to the Out of Hours service.
If you call the Practice telephone number when we are closed you will be given the option to be transferred to 111,
however your service provider will charge you for this call.
The Out of Hours services is available for urgent consultations which cannot wait until your GP surgery is open.
NHS Direct
In the case of urgent need when the practice is closed you can call NHS Direct (www.nhsdirect.nhs.uk/) (0845 4647)
to speak to a triage nurse. Your needs will be assessed and advice offered or arrangements made for you to see a
doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency.

NHS Walk-in Centre
There is a Walk-in Centre at 63 Monks Road, Lincoln LN2 5HP. It is open Monday to Sunday 8.00 am - 8.00 pm.
The centre is run by a team of experienced nurses and GPs. You do not need to make an appointment.

Appointments
All appointments can be made by telephoning 0844 815 1384 and selecting the appropriate option.
We do not accept appointment requests by email.
You may consult any of the Doctors in the Practice, but we would encourage patients to see the same Doctor for
ongoing problems to enable continuity of care.
If you have an urgent problem, you will be offered an appointment on the same day, but this will not necessarily be
with the Doctor of your choice.
Please book separate appointments for each member of the family attending Surgery.
Cancelling Appointments
Should you need to cancel an appointment please telephone the surgery on 0844 815 1384 before your
appointment time so we can offer your slot to another patient.
Home Visits
If possible please try to telephone reception before 10am if you require a home visit.
House visits are only available for patients who are genuinely housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit.
There are also better facilities for examining and treating patients at the surgery.
Emergency Visits
Emergencies will be dealt with immediately by the Duty Doctor. Please stress that the call is "an emergency".
Telephone Consultations
If you wish to speak to your Doctor on the telephone, it will normally be possible to do this by arrangement with the
Receptionist. It would be helpful, in assessing the urgency of the call and also ensuring that the Doctor has the
relevant information available, if you could indicate to the Receptionist the nature of your call when you ring.
However, sometimes interruptions during surgery can be upsetting both for the Doctor and the patients they are
seeing. For this reason, the Receptionists are asked not to disturb consultations with non-urgent telephone calls. A
more convenient time for you to ring will be suggested.
Practice Information
Disabled Access
We aim to be fully accessible for our disabled patients and have an ongoing programme to comply with the
Disability Access Act. Disabled patients will not find any difficulty in obtaining access to the Surgery where all
main services are provided on the ground floor. A toilet for disabled people is also available.
Prams, Buggies and Pushchairs
To comply with fire regulations, these should be left in the space provided by the front door.
Car Park
A car park is available for the use of patients attending the Surgery, the staff and the Doctors. Please note that
persons using the footpaths and car park do so at their own risk. No responsibility is accepted for injury to
pedestrians or for any loss or damage caused to vehicles, their contents or accessories, or any injury caused by or to
owners or occupants of such vehicles whilst entering or leaving the premises .
Medical Fund
Over the years, our patients have helped us enormously with charitable donations and bequests for the purchase of
medical equipment. If any patient would like to help us by making a donation to the Minster Medical Practice,
would they please contact the Practice Manager or a Partner.
Repeat Prescriptions
We accept repeat prescription requests online.
To use this service you will need to ask at Reception where an account will be set up for you. Once you have set up
a password you will be able to order repeat prescriptions online.
Patients on long-term tablets or medicines agreed by their Doctor may obtain repeat prescriptions on a regular basis
without seeing the Doctor each time. Sometimes, however, you may be asked to make an appointment so that your
medication can be reviewed.
New patients on regular treatment will be asked to make an appointment with the Doctor soon after registering to
discuss their medication.
If your Doctor gives you a repeat prescription, you will also receive a printed counterfoil. Please keep this until you
require a further supply of medicine. Then, either leave the counterfoil in the special repeat prescription box at
the side of the reception desk,post it to us or order online.
Please allow at least 2 working days for your prescription to be processed.
When your prescription is ready, you can collect it from the Surgery or, alternatively, we can post it to you - but we
do ask that you provide us with a stamped addressed envelope for this. If you are genuinely housebound, the local
chemists are happy, if you make prior arrangements with them, to collect prescriptions from the Surgery and deliver
your medication free to your home. Could you please also notify us by writing this on the counterfoil.
We do not take repeat orders for medication over the telephone.

New Midnight Dispensing Pharmacy
This can be found at Boots chemist at the Carlton Centre on Outer Circle Road. They are open Mon-Sat until
midnight and Sun until 4:30 pm.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
These charges apply in England only. In Northern Ireland, Scotland and Wales
prescriptions are free of charge.




3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 14 items
in 12 months, you may find it cheaper to buy a PPC.



Our Nurse Practitioner is available by appointment every day to see patients with any newly presenting problems.
She is able to assess, diagnose, refer and prescribe medication if required (over 1 year olds only).
Our practice nurses are available by appointment for general nursing duties, e.g. dressings, immunisations, ear
syringing etc, which the Doctor may arrange.

Though not employed by the Practice, these professionals liaise with surgery personnel and have full use of our
facilities to provide additional services for our patients.

regarding all aspects of their care and health.

If no response you can contact The Community Midwifery Office on 01522 573772 where you can leave a message.
Our midwife provides the majority of antenatal and postnatal care and is at the surgery on Thursday and Friday
mornings. She works with a small team of midwives practicing in the North of the City.

District Nurses
The District Nursing team offer care in the homes of those patients unable to attend the surgery. They are based at
Ravendale Health Centre and can be contacted on 01522 533331.
Non NHS Services
Non NHS/Private Services
Certain services provided by your Doctor are not covered by the NHS, e.g. completion of private medical insurance
forms etc, for which you may be asked to pay a fee. A list of the fees we charge is displayed in the Reception area
but please ask the Receptionist for details if you are in doubt.
All forms for completion by the Doctor should be handed to the Receptionist and up to 10 days allowed before
collection. It should be noted that there may be a charge for certain travel vaccinations.
Test Results
Test Results
Please call between 2:00 pm and 5:00 pm to enquire about your test results as our reception staff will have more
time to deal with your request between these times.
Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test
results to the person to whom they relate unless that person has given prior permission for the release of this data or
they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the practice.
It is your responsibility to check your results and to make an appointment to discuss them with your doctor if you
are advised to do so.
Blood Tests
Blood Tests
A blood test is when a sample of blood is taken for testing in a laboratory. Blood tests have a wide range of uses and
are one of the most common types of medical test. For example, a blood test can be used to:



assess your general state of health
confirm the presence of a bacterial or viral infection
see how well certain organs, such as the liver and kidneys, are functioning
A blood test usually involves the phlebotomist taking a blood sample from a blood vessel in your arm. and the usual
place for a sample is the inside of the elbow or wrist, where the veins are relatively close to the surface. Blood
samples from children are most commonly taken from the back of the hand. The child's hand will be anaesthetised
(numbed) with a special cream before the sample is taken.

X-Ray
X-Ray
An X-ray is a widely used diagnostic test to examine the inside of the body. X-rays are a very effective way of
detecting problems with bones, such as fractures. They can also often identify problems with soft tissue, such as
pneumonia or breast cancer.
If you have a X-ray, you will be asked to lie on a table or stand against a surface so that the part of your body being
X-rayed is between the X-ray tube and the photographic plate.
An X-ray is usually carried out by a radiographer, a healthcare professional who specialises in using imaging
technology, such as X-rays and ultrasound scanners.

The doctors welcome new patients who live within our practice area.
When you apply to join this Practice you will be asked to complete a confidential questionnaire, and if you
are on repeat medication you will be invited to attend an introductory appointment with one of the
Doctors.

Practice Area
The Practice area extends north of the River Witham to the Ring Road. It DOES NOT INCLUDE Monks Road and
the adjacent streets from the River Witham to Lindum Avenue/ Sewell Road. It DOES INCLUDE the villages of
Burton Waters, Burton, Riseholme, and North Greetwell. We also accept patients from the villages of Nettleham,
Scothern, Sudbrooke and North and South Carlton.
Practice Policies
Confidentiality & Medical Records
The practice complies with data protection and access to medical records legislation. Identifiable
information about you will be shared with others in the following circumstances:



To provide further medical treatment for you e.g. from district nurses and hospital
services
To help you get other services e.g. from the social work department. This requires your
consent
When we have a duty to others e.g. in child protection cases anonymised patient
information will also be used at local and national level to help the Health Board and
Government plan services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us
know.
Reception and administration staff require access to your medical records in order to do their
jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.
Freedom of Information
Information about the General Practitioners and the practice required for disclosure under this act
can be made available to the public. All requests for such information should be made to the
practice manager.
Access to Records
In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients
may request to see their medical records. Such requests should be made through the practice
manager and may be subject to an administration charge. No information will be released
without the patient consent unless we are legally obliged to do so.
Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a
genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly,
and as amicably as possible.
To pursue a complaint please contact the practice manager who will deal with your concerns

Travel Vaccinations
If you require any vaccinations relating to foreign travel you need to make an appointment with
the practice nurse to discuss your travel arrangements. This will include which countries and
areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you
travel - as a second appointment will be required with the practice nurse to actually receive the
vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second
appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Most travel vaccines are ordered on a private prescription and these incur a charge over and
above the normal prescription charge. This is because travel vaccination is not included in the
services provided by the NHS and therefore is a chargeable service.

Summary Care Record
There is a new Central NHS Computer System called the Summary Care Record (SCR). The
Summary Care Record is meant to help emergency doctors and nurses help you when you
contact them when the surgery is closed. Initially, it will contain just your medications and
allergies.
Later on as the central NHS computer system develops, (known as the ‘Summary Care Record’ –
SCR), other staff who work in the NHS will be able to access it along with information from
hospitals, out of hours services, and specialists letters that may be added as well.
Your information will be extracted from practices such as ours and held on central NHS
databases.
As with all new systems there are pros and cons to think about. When you speak to an
emergency doctor you might overlook something that is important and if they have access to
your medical record it might avoid mistakes or problems, although even then, you should be
asked to give your consent each time a member of NHS Staff wishes to access your record,
unless you are medically unable to do so.
On the other hand, you may have strong views about sharing your personal information and wish
to keep your information at the level of this practice. Connecting for Health (CfH), the
government agency responsible for the Summary Care Record have agreed with doctors’ leaders
that new patients registering with this practice should be able to decide whether or not their
information is uploaded to the Central NHS Computer System.
For existing patients it is different in that it is assumed that you want your record uploaded
to the Central NHS Computer System unless you actively opt out.

Patient Advice
Patient Advice and Liaison Service (PALS)
(www.pals.nhs.uk/) The NHS employs over a million staff in thousands of locations. It is a large
and complex organisation providing a broad range of services. It is not surprising that sometimes
you or a loved one may feel bewildered or concerned when using the NHS. And this can be at
times when you are feeling at your most vulnerable and anxious.
So, what should you do if you want on the spot help when using the health service? The NHS
expects all members of staff to listen and respond to you to the best of their ability. But
sometimes, you may wish to talk to someone employed especially to help you. The Patient
Advice and Liaison Service, known as PALS, has been introduced to ensure that the NHS listens
to patients, their relatives, carers and friends, and answers their questions and resolves their
concerns as quickly as possible.
PALS also helps the NHS to improve services by listening to what matters to patients and their
loved ones and making changes, when appropriate.
What does PALS do?
In particular, PALS will:







Provide you with information about the NHS and help you with any other health-related
enquiry
Help resolve concerns or problems when you are using the NHS
Provide information about the NHS complaints procedure and how to get independent
help if you decide you may want to make a complaint
Provide you with information and help introduce you to agencies and support groups
outside the NHS
Inform you about how you can get more involved in your own healthcare and the NHS
locally
Improve the NHS by listening to your concerns, suggestions and experiences and
ensuring that people who design and manage services are aware of the issues you raise
Provide an early warning system for NHS Trusts and monitoring bodies by identifying
problems or gaps in services and reporting them.

years doing hospital training in London and Kent followed by one year's GP
training in Scarborough. He joined the Practice in August 1995. His medical
interests include IT, child health surveillance and minor surgery. He achieved
Fellowship by Assessment in 2000. He is the Practice Lead for Clinical
Governance, Prescribing, IT, and the Quality and Outcomes Framework. He is also
a GP Tutor for medical students based at the University of Nottingham Medical
School.

Graduated from Liverpool Medical School in 1989, having enjoyed an active role
in medical student life, being Treasurer of the Society in his fourth year and
Captain of the Medical School Football Team for three years. Following House
Officer jobs, he moved to Macclesfield in Cheshire for his vocational training and
took a year out to work in Australia prior to finishing his GP training. Since being
in Lincoln, he has taken up the position of Club Doctor to Lincoln City Football
Club, where he is now in his tenth season, and has graduated from Nottingham
University with an MSc in Sports Medicine. Within the Practice, he is responsible
for finance and complaints, with a shared responsibility for personnel and is also
responsible for the Practice appointment system. Clinically, he has interests in
asthma, diabetes and minor surgery in addition to Sports Medicine. He is also a GP
with a Special Interest in Orthopaedics.

in the Lincoln Vocational Training Scheme in 2003. He then worked as a Salaried
Principal in two local Practices before joining Minster Medical Practice as a Partner
in August 2004. He has recently completed a Postgraduate Certificate in Medical
Education and is an approved GP Trainer. He is mentor to the Nurse Practitioner
and enjoys the wide variety of General Practice, particularly orthopaedics and
minor surgery.

Graduated from The Royal Free Hospital School of Medicine in 1981 and
completed her postgraduate training in Inverness. She moved to Lincoln in 1985
and worked as a Locum for various practices, including the Minster Practice, until
1989. After three years of locum work in Cornwall, she returned as a part-time
Partner in 1992. In 1999 she achieved Fellowship by Assessment and in 2000,
DFFP (The Diploma of Faculty of Family Planning and Reproductive Health). Her
interests include gynaecology, family planning and teaching. In 2002, she was
approved as a Trainer and acted as an FBA advisor to a local GP.

Graduated from University Complutense in Madrid in 1989 and moved to the UK
where she initially gained wide postgraduate experience in surgical jobs, and
particularly in orthopaedics. She spent one year as Anatomy Demonstrator at St
George's Medical School in London and passed Primary FRCS in 1995. She then
joined The Greenwich Vocational Training Scheme for General Practice in South
East London. She completed her training in Lincolnshire in 2001 after placements
in psychiatry in Lincoln and GP Registrar at Market Rasen. She joined Minster
Medical Practice as a GP Assistant and became a Salaried Doctor in 2003. She
achieved ALS Provider Certificate in 1998 and DFFP in 2001. She ran a teenage
Family Planning Clinic at Market Rasen. Her interests include gynaecology, family
planning, orthopaedics and minor surgery.

problems. She is able to assess, diagnose, refer and prescribe medication if
required (over 1 year olds only).

Available by appointment for general nursing duties, e.g. dressings, immunisations, ear
syringing etc, which the doctor may arrange. In addition they run specialist clinics, e.g.

together with various health promotion activities. They also assist the doctors with various
clinics.

Our receptionists are your first contact with the Practice. It is their job to advise and guide you and
any information they receive is treated in the strictest confidence. If you are willing to provide them
with details of your enquiry, they will then be able to assist you in the use of our range of services.
Confidentiality
Our staff are bound by the same rules of confidentiality as the Doctors and Nurses. They have a
difficult job to do, especially in dealing with large numbers of patients who visit and telephone the
surgery each day. They have to try to keep both the Doctors and yourselves happy and in order to do
so, may have to ask what appear to be unnecessary questions. Please remember that they are only
doing what they are asked to do by the Doctors and, by providing the information they need, you are
helping them to help you.
Access to Medical Records
Access to medical records can be arranged within the scope of the relevant laws - details from the
Practice Manager. The request for copies of medical records needs to be in writing and charges may
be made for the provision of this facility.

Kay provides the majority of antenatal and postnatal care and is at the surgery all day
Thursday and Friday mornings. Kay works with a small team of midwives practicing in
the North of the City.
