Buy as much as you like!!! Please send out your ORDER FORM with your HK CURRENCY CHEQUE!!!

Notes
@1 Because of limited administration strength, we prefer you collecting the items from us (by yourself, your friend
or FA Representative). We suggest that name of the “anticipated collector” be entered in Section 1.
Delivery Charges
@2 In order to save limited financial resources for our committed objectives, all incurred postal fees, packaging
and transport (from OSA’s office to local postal office) have to be recovered on all “mail orders”.
@3 Please sum up total “Gross Weight” of your purchase (anticipated wrappings required by postal offices already
included), and compute corresponding delivery charges into your payment. Each mail order has to be within 20
kg.
@4 We anticipate all our manpower will be occupied by GJC activities before mid December 2004 and all mail
orders will be delivered in 2005!! All payments will have to be cashed before delivery to facilitate GJC
account finalization.
@5 The purchase would deemed to have been received by you when the postal office indicate to OSA or confirm
with OSA that the parcel has been delivered and received.

@6 We prefer Hong Kong currency payment. If other currency is to be used in your payment, please include
HK$60 bank charges per each foreign currency cheque and exchange at US$1 = HK$7. Equivalent
exchange rate should be used for other currency.
@7 Cheque should be crossed, payable to “Q.E.S.O.S.A. Ltd.” and mail with the completed Order Form to
“QESOSA Ltd. GPO Box 13119, Hong Kong”.

@10 “Receipt” will be sent to you by email, fax or postal (if you don’t have email address or fax number). We may
have to check your Identify during the collection. If you authorize representative to collect your purchase,
please complete the authorization section of the “Receipt” and ask your representative to bring along
photocopy of your ID. We may have to record identity of your representative during collection.
