Situation Summary

1.

Red-tagged buildings include: a.

412 Broadway – one night club (Showgirls) b.

1035-1039 Montgomery – 3 residential units c.

1041-1045 Montgomery – 3 residential units d.

1047-1051 Montgomery – 3 residential units

2.

Yellow-tagged buildings include: a.

455 Vallejo – 45 residential condominium units. No access allowed to 12 units at southeast portion of building (portion of building adjacent to patio deck), and no access to wood framed deck outside laundry room. No access to upper floor units within arc of crane boom when loads are being lifted by crane. b.

409 Vallejo – 2 residential units. No access to rear decks. c.

426-430 Broadway – 14 residential units, one restaurant (Helmand). No access allowed to restaurant, nor to 4 residential units at rear portion of building, except for possession retrieval. Occupancy of 9 residential units at front portion of building is allowed. d.

432-434 Broadway – 3 residential units (multiple tenants due to individual room rental), one restaurant (Green Garden). No access to second floor roof and rear yard. e.

438-440 Broadway – 35 residential units, one night club (Dolce). No access to second floor roof, main roof, and rear yard; limited access to storage area at rear of Dolce (440

Broadway). f.

1053-1057 Montgomery – 3 residential units. No access to rear stairs, rear yard, rear deck and roof

3.

Residents in all red-tagged and no-access portions of yellow-tagged buildings may remove all belongings, including furnishings, provided work is performed under the continuous observation by a registered geologist, engineering geologist, or DBI personnel by appointment. All such work to be done during daylight hours (7:00 am to 6:00 pm).

4.

At this time, DBI considers the on-going and future work related to the rockslide as a typical job site. Appropriate permits are required.

Red-tags will remain in the southeast corner of 455 Vallejo pending further engineering and geologic investigation. (Unit numbers Garden A, Garden B, Garden

C, 101, 102, 103, 201, 202, 203, 302, 303, PH8). Engineering and geologic investigations should include verification of the stability of the rock upon which the concrete retaining wall sits, submission of engineering report showing the remaining portion of concrete retaining wall is adequately supported for vertical and horizontal loads, and verification that the existing drilled piers are adequately supported by the rock and soil. b.

Work with 412 Broadway and 426-430 Broadway owners to remove rock from behind their buildings. Red-tag to be removed from 412 Broadway pending submittal and approval of building permit for remediation work. c.

Work with Montgomery Street property owners to remove falling rock hazards. d.

Work with affected parties towards permanent stabilization (e.g., shotcrete or netting and rock bolts, etc.) of the hill side.

Event Log

1.

On the morning of 2/27/07, an approximate 75 feet wide section of the rear yard cliff face behind 455 Vallejo Street slid down onto the properties fronting Broadway and

Montgomery approximately 80 feet below. The rear yard of 455 Vallejo Street consists of a vertical cut cliff face above the rear yards of the lots fronting Broadway. Rock and boulder debris from the rockslide, believed to be in hundreds of cubic yards, apparently slid off the cliff face and piled up on rear yards of these buildings along Montgomery and

Broadway. At a few locations of these Broadway buildings, the debris either pushed or is pushing against the rear walls of these buildings.

2.

SFFD and SFPD arrived on scene at approximately 03:20.

3.

DBI was notified shortly after 05:00 and immediately dispatched senior inspector, engineer, and geotechnical engineer consultant. Senior inspector on scene at 05:45 and proceeded to evaluate rockslide and damage to buildings along Broadway. His preliminary assessment was at the three buildings along Broadway requiring red tags – unsafe to occupy. This was confirmed 15 minutes later by the geotechnical engineer consultant and Deputy Director. Vallejo property was red tagged and three properties on

Montgomery were initially yellow-tagged and later red-tagged.

4.

DBI, SFPD and SFFD set up joint incident command on Broadway.

5.

DBI coordinated events with DEM through regular scheduled briefings.

6.

DBI worked with SFFD and SFPD to recommend that Broadway be opened to rush hour traffic.

7.

DBI assumed sole incident command on 2/28/07. DBI Emergency Command Vehicle

(ECV) dispatched and stationed at Broadway and Montgomery.

8.

The rockslide from 455 Vallejo fell onto the following properties: 412 Broadway, 426-

430 Broadway, 432-434 Broadway, 1035-1039 Montgomery, 1041-1045 Montgomery,

1047-1051 Montgomery.

9.

Red-tagged 7 buildings include:

10.

The rockslide affected a total of 71 units of housing and displaced approximately 120 residents in the red-tagged buildings.

11.

A M4.2 earthquake occurred at 3/1/07 20:40 with an epicenter 2km northeast of

Lafayette. The site was visually surveyed after the earthquake and the rock face appeared to be stable.

12.

At 3/2/07 11:40, the status for the following properties has changed; however, red tags remain.

Entry to these properties for possession retrieval (including furniture) is allowed during daylight hours (06:00 to 19:00). Entry is controlled by Police. Verification of tenancy and/or ownership must be provided prior to access. Tenants and/or owners must accompany movers.

13.

March 3, 2007 06:30 begin implementation of hazard mitigation program by Granite

March 3, 2007 10:30, DBI is coordinating access for Granite with other agencies. a.

Police to provide escort for crane b.

PUC has marked off water lines on street c.

PG&E has turned off gas to 455 Vallejo at the street d.

DPW has photo documentation of the street, retaining walls, and tress. There is potential for damage to the trees due to the crane boom and counterweight e.

DPW to provide barriers restricting pedestrian access in front of 455 Vallejo (south side) f.

15.

March 3, 2007, Red Cross closed the shelter at 1199 Mason Street. Residents should contact Red Cross at 427-8010 for assistance.

March 3, 2007 16:00, DBI is not allowing any access to all red-tagged buildings while mitigation work is proceeding. DBI is allowing limited access when mitigation work does not present any danger to building occupants.

March 3, 2007 18:00, Granite made their first slab lift.

18.

March 4, 2007 07:00, Granite held their safety meeting and are ready to proceed with mitigation work. DBI is allowing limited access when mitigation work does not present any danger to building occupants.

19.

March 5, 2007 16:15, a large piece of rock (approximately 12 feet x 20 feet) fell during mitigation work. Work was stopped.

20.

March 5, 2007 14:00, yellow tag posted on 438-440 Broadway. No access to rear yard, rear deck, or roof. (448 Broadway which is part of the same building is not affected.)

March 5, 2007 16:30, meeting between Granite (and consultants), DBI (and consultants), and owners of 455 Vallejo, 1047-1051 Montgomery, 1041-1045 Montgomery, and 1035-

1039 Montgomery regarding the status of the hazard mitigation efforts. a.

Has scope of work been completed? b.

Current contract between Granite and 455 Vallejo expires at 5:00pm, 3/6/07. Owners agreed to meet again at 11:00am on 3/6/07 to discuss and develop plan for the next steps.

March 7, 2007 12:40, meeting between DBI (plus Frank Rollo and Lou Gilpin), owner of

455 Vallejo, Granite (plus Cotton Shire, Sure Engineering) to discuss the following: a.

DBI needs geological letter and engineering letter from 455 Vallejo indicating that building may be occupied or partially occupied b.

DBI needs geological letter indicating scope of work required to ensure the stability of the existing concrete retaining wall c.

445 Vallejo to present DBI with plan for partial re-occupancy and how such occupancy will affect the remainder of work d.

Granite currently working on scaling of rock at top of slope and removal of material from behind the existing concrete wall e.

DBI needs scope of work from Sure Engineering regarding additional remediation work f.

Sure Engineering to submit plans and calculations for permanent fix to concrete retaining wall and patio area

March 7, 2007 14:00, caution tape applied to rear door of Dolce (440 Broadway) and VIP area. No access to these areas during mitigation work. Access to these areas may be allowed during evenings (when work is not in progress) based on geological consultant recommendations.

March 7, 2007 15:30, California Office of Emergency Services geologist, Ed Bortugno, site visit.

28.

March 7, 2007 17:00, additional patio slab was chipped out and removed. Loose rock and soil was removed at the top of slope. The concrete wall footing was verified via trenching behind the existing concrete retaining wall. Tarps applied over deck and slope for rain protection.

29.

March 8, 2007, additional loose rock and soil removed at the top of slope.

30.

March 9, 2007, re-evaluated 426-430 Broadway and 455 Vallejo and outlined requirements to owners for red-tags to be changed to yellow.

31.

March 10, 2007: excavation of soil behind 455 Vallejo retaining wall in progress using small Caterpillar backhoe-type tractor with stated objective of excavating to top of retaining wall footing and laying back the soil at approximately 1:1 slope. Spoils from excavation operations are lifted by crane to Vallejo Street. Also observed excavation of

“approach pit” in the patio area adjacent to the patio stair and the side of the building.

Plan and objective for approach pit is unclear.

32.

March 10, 2007: with the re-occupation of portions of 455 Vallejo, Tony

Martinez/Granite was verbally advised by HZ/DBI that no occupancy of the uppermost story units is permitted while the crane is in operation lifting loads over the building.

Granite to advise occupants of affected units and coordinate as necessary.

33.

March 10, 2007 16:50: Construction action at 455 Vallejo patio area. Caterpillar excavator overturned while excavating behind retaining wall. No reported injuries.

Crane used to right overturned equipment.

34.

March 10, 2007 17:30: Posting of 426-430 Broadway revised to yellow tag status, to allow re-occupancy of nine units in front portion of building. Four units at rear portion of building and restaurant remain posted with red tag and unsafe to occupy.

35.

March 10, 2007 19:00: SFPD Central Station Sgt. Chiu advised police units no longer required by DBI on Broadway St.

36.

March 11, 2007: At 455 Vallejo, temporary fencing and plywood door being installed at patio balcony area to restrict access to the patio area.

37.

March 11, 2007: Excavation of soil behind retaining wall continues and revealed existing concrete block apparently acting as a deadman anchor tied back to retaining wall with bare rebar tension ties. Little signs of corrosion of rebar. Concrete deadman is T-shaped and located approximately 12 ft from cut end of wall and approximately 4 ft from face of wall. Carl Chan indicated the deadman is likely insufficient by inspection and will be removed with the sawcutting of the retaining wall.

38.

March 11, 2007: Exploratory “approach” pit has been excavated to several feet depth and several feet wide adjacent to the grade beam under the building adjacent to the base of the stairs to the patio.

39.

March 11, 2007: Posting of 432-434 Broadway revised to yellow tag status, to allow reoccupancy of the entire building. No access to second floor roof and rear yard.

40.

March 11, 2007 16:20: De-mobilization of DBI ECV.

Incident Objectives for this operational period:

Monitor buildings and rock face. DBI engineer to check progress at top of slope every hour during mitigation work.
