T emperature of 99.4 or higher during the past 24 hours.
Vomiting or diarrhea during the night or before school.
Contagious diseases such as chickenpox, strep throat, pink eye, or any unexplained rash.
Students may return to school after 24 hours of antibiotic therapy for strep throat or pink eye.
Prescription Medication: These must be in the original container with the original pharmacy label that has complete instructions including the name of the medication, dosage and times to be given. The physician’s name must be on the label. There must also be a written note from the parent or guardian with signature. In this note, a list of any allergies the student has or No Known Drug Allergies if the student does not have any must be included.
Over-the-Counter Medication: There must be a written note giving permission before any over-the countermedication can be given. This must include the name of the student, name of the medication, the dosage, intervals or time and instructions. This must also have a parent’s signature. This form can be kept in the office for the school year but expires at the end of each year.
Inhalers: All inhalers must stay in the secretary’s office unless we have a note from the student’s physician stating that the child is capable of self administration.
All medication should be taken to the secretary at the beginning of the school day. If the medication is to be taken home at the end of the school day, it will only be released to the student’s parent/guardian or to the student that brought it.
