I. GOAL – State the purpose of this grant request as it relates to your organization's mission and long term goals. II. OBJECTIVE – Describe the project you are proposing and its anticipated timeframe. Include its expected outcome, how it will impact the community your agency serves, and the number of people who will benefit. In addition, describe the impact on the communities of Berkeley Heights, Chatham, New Providence, and Summit, and the number served from those communities. III. ACTION PLAN – Outline the implementation plan that will enable you to reach your objective, following the outline below: A. Personnel/Authority 1. Who in your organization is responsible for overseeing this project? Include name, title and phone number. 2. Who will be involved with the project on a day-to-day basis? a.
