This program is designed to allow you to take college-level courses for high school and college credit. You are responsible for knowing and understanding all information in this agreement. By being enrolled in the Wright State University (WSU) Dual Enrollment Program (DEP), you will receive: 1. Opportunity to earn concurrent high school and college credit, giving you a head start in college. 2. College credit at a reduced rate. 3. Academic Support and Advising. DEP students will have access to academic support via services currently provided to all WSU students. These services include access to academic advising via WSU’s University College, the WSU Student Academic Success Center, and other student academic success resources. 4. A WSU student ID! You will be a part-time WSU student and will be able to obtain a Wright State University “Wright One ID card” that allows access to WSU resources such as the library and computer labs. As a student in the DEP program at Hayes High School, you will be expected to: 1. Attend class. Excessive absences may result in your being withdrawn from the DEP program without a refund. 2. Prepare for class. It is expected that you will complete all homework and/or reading assignments. 3. Participate in class. During class it is expected that you will take notes, be attentive, and participate in class discussions. 4. Maintain a grade point average of 2.00 (minimum grade of C) or higher in all DEP classes. 5. Attend Orientation: DEP students and parent/guardian will attend an orientation prior to the start of DEP classes, intended to inform and equip students for college. Attendance is required for all DEP participants. 6. Tuition and Registration Fees: For 2011-12, students participating in the Hayes High School - Wright State University DEP will qualify for the discounted WSU tuition charges for DEP courses. Students will be required to pay Delaware City Schools $370.00 (tuition/fees = 4 credits @ $90.00 per credit + one-time $10.00 DEP fee). 7. Books: Hayes High School will provide the DEP textbook(s) for each student with the expectation that the texts are returned to the school at the end of the academic semester. Or, students may purchase their own copies of the textbooks from any vendor of their choice. 8. Inform your Hayes High School counselor and WSU of any PSEO and/or AP credit already earned. You will not receive credit for duplicate courses. By signing this agreement with Wright State University, I am requesting to register for Dual Enrollment classes and promise to assume financial responsibility for all charges billed to my student account including, but not limited to tuition, general fees, late fees, and fines (aka: Student Obligations). By signing this form, I acknowledge that I have read and understand the following provisions: If my school district fails to pay Wright State University by the end of the term, I assume responsibility for paying the amount the school district originally promised to pay on my behalf. Additional late payment fees will be assessed throughout the term if I fail to pay my Student Obligations during the term. A “HOLD” will be placed on my records if my Student Obligations remain unpaid after the official refund period ends for the term which will prevent my ability to register for future terms, adjust class schedule for the current term, receive an official university transcript and have degree status confirmed by the university. All costs, expenses, late fees, and attorney fees related to the collection of my Student Obligations will be paid by me to the full extent permitted by law. Ohio law requires that non-resident fees be assessed to male Ohio students who are between the ages of 18 and 25 and have not registered with Selective Service. I further agree that the university has my consent to notify my parents, my school district superintendent or designee, the state superintendent of public instruction, and the Chancellor of the University System of Ohio of my enrollment and grades in classes at Wright State University. ______________________________________________________//___________________________________________________________ Student Signature Date Parent/Guardian Signature Date
