An Indemnity Bond to be executed on Non judicial Stamp Paper of requisite value affixing the present
photograph of the indemnifier(s) and to be counter guaranteed by a reputed person duly affixing the
present photograph of the Guarantor thereon and attested as per the enclosed proforma along with the

An Affidavit to be sworn before a Magistrate or a Notary Public on Non-judicial stamp paper of
requisite value affixing the present photograph of the declarant/s as per the enclosed proforma along
with the Notarial Certificate.
Consent letter from the other legal heir(s) if any, as per the enclosed proforma duly attested along with
the Notarial Certificate.
The enclosed Transmission Form duly completed .
Photocopy of the death certificate of the deceased shareholder duly attested. In case the death
certificate is in vernacular, kindly send us an English translated copy of the same duly attested.
The original share certificate.
Photocopy of the Will , if any ,of the deceased shareholder duly attested.
Bank account details
Unencashed dividend warrants if any.
Photocopy(ies) of the PAN of the legal heir(s) in whose name(s) shares in question are to be
transmitted.
Specimen signature(s) of the Indemnifier(s) and Guarantor duly attested by the Manager of the Bank on
a plain paper affixing the photographs of the Indemnifier(s) and Guarantor thereon .
