DISCHARGE FROM ACTIVE
DUTY WITH SEVERANCE PAY

The PEB has made a determination of UNFIT to perform the duties of office,
grade, or rank due to a physical disability, and separation from the Marine
Corps with severance pay and a DoD disability rating of ____%.
Command will submit 11060 Form to local Disbursing Officer to process
Marine’s final separation pay to include severance pay.
Contact the local Veteran’s Affair (VA) office to enroll in the VA and for
information and action required to determine if there are any entitlements to a
VA disability rating.
TAMPS/DTAP training must be completed prior to separating.
Report to DEERS office to be issued a temporary ID card. Tricare
Coverage and military privileges will expire 180 days after discharge date.
Command will complete discharge orders and DD214.
