‫المقررة دون أي مطالبة إضافية‪.‬‬

‫يلتزم من يرسو عليه العطاء بتورية اأصناف المطلوبة وفو الشوروا البوابقة ودة موةة شو ر ) مون‬
‫تاريخ تكليفه رسمياً‪.‬‬
‫تلتزم الجامعة اإلسدمية بةفع جميع استحقاقات من يرسو عليه العطاء‪ -‬ارج الوان‪ -‬بعة االنت واء مون‬
‫تنفيذ كامل ما ورد ب ذا العطاء‪.‬‬
‫في حاة تف ير من يرسو عليه العطاء في إتمام بنود هذا العطاء في مواعيةه المحةدة يلتزم بوةفع غراموة‬
‫تف ير مقةارها ‪ ) $000‬مائة دوالر أمريكي عن كل يوم تف ير‪.‬‬
‫إذا زادت مةة التف ير في التورية عن أسبوعين يح للجامعة اتخاذ كافة اإلجراءات التوي تراهوا مناسوبة‬
‫بح المتع ة ويعتبر أي إجراء تتخذه الجامعة قانونيا ً دون أن يكون له ح االعتراض‪.‬‬
‫جميع اأسعار يتم تقةيم ا بـالةوالر اأمريكي ‪.$‬‬

‫الصيانة الخارجية وبتقةيم الج از البةيل إذا زادت مةة الصيانة عن أسبوا من تاريخ اإلشعار‪.‬‬
‫تحتفظ الجامعة بحق ا في رفض أي عرض دون بيان اأسباب واستةراج عروض من الخارج فوي حواة‬
‫رفض العروض المقةمة‪.‬‬
‫من ح الجامعة زيادة الكمية المطلوبة من اأج زة عن الكمية الموضحة في عرض البعر‪.‬‬
‫الجامعة غير ملزمة بفقل اأسعار‪.‬‬

‫التعليمات الخاصة بالعطاء‪-:‬‬
‫‪ )1‬يمكن للمتع ةين الراغبين في المشاركة ب ذا العطاء أن يتقةموا للحصوة على نبخة كاملة من ويائ‬
‫العطاء مقابل دفع المبلغ المقرر من الجامعة اإلسدمية بغزة ‪.‬‬
‫‪ )2‬ينبغي ع لى كل من يرغب د وة هذا العطاء أن يقوم بةراسة دقيقة لكافة المواصوفات وجوةاوة الكميوات‬
‫والشروا العامة وأن يتف م ماهيت ا والظروف المحيطة ب وا وسوائر العوادات المحليوة والرسووم واأموور‬
‫التي ل ا عدقة ب ا‪.‬‬
‫‪ )3‬يلتزم المتع ة بتقةيم عروض اأسعار على نفس نموذج كراس العطواء المقوةم مون الجامعوة وبامكانوه أن‬
‫ييوويإ إليووه المبووتنةات والويووائ التوضوويحية اأ وورى الدزمووة للعطوواء علووى أن تكووون كافووة اأورا‬
‫والمبتنةات موقعة ومؤر ة في اأماكن المحةدة لذلك‪.‬‬
‫‪ )4‬تعتبر كافة اأسعار التي ييع ا المتع ة أمام كافة بنود العطاء على أن ا القيمة الكاملة والشواملة إلنجواز‬
‫المطلوب في ذلك البنة وأن ا تشمل كذلك أرباح العطاء وتعوييوه عون أي التزاموات أ ورى قوة يتحمل وا‬
‫المتع ة وفقا ً ل ذا العقـة‪.‬‬

‫إلووى دائوورة اللوووازم والمشووتريات بالجامعووة اإلسوودمية فووي موعووة ال يتووف ر عوون البوواعة الواحووةة ر وور يوووم اأحووة‬
‫المواف ‪2212/1/31:‬مـ‪ ،‬وتفتح المظاريإ الباعة الواحةة والنصإ ر ر نفس اليوم ويمكن للجامعوة اإلسودمية‬
‫تفجيل موعة تقةيم العروض باشعار طي يبعث إلى كل مون المتع وةين الوذين حصولوا علوى ويوائ العطواء ولون‬
‫تنظر الجامعة)في أي عطاء يصل ا بعة الوق المحةد لذلك‪.‬‬
‫‪ )5‬يعتبر العورض المقوةم ملزموا ً للمتع وة وال يجووز سوحب هوذا العورض بعوة تقةيموه ويظول العورض ملزموا ً‬
‫للمتع ة الذي تقةم به لفترة ‪ 3‬ش ور ابتةاء من أ ر موعة تم تحةيةه لتبليم العروض ‪.‬‬
‫‪ )6‬ال يجوووز إد وواة أي تعووةيل أو إجووراء أي تبووةيل فووي نموووذج العوورض للعطوواء أو أي موون ويووائ العطوواء‬
‫المرفقة به من قبل المتع وة باسوتاناء تعب وة المواقوع المطلووب تعب ت وا وإذا أجورى المشوترة أيوا مون تلوك‬
‫التعةيدت أو التبةيدت أو إذا أ ل بفي من هذه التعليموات فيجووز للجامعوة اإلسودمية ) إهمواة عرضوه‬
‫وإذا أراد المتع ووة الووذي يرغووب فووي تقووةيم بعووض الشووروا أو التحفظووات التووي يراهووا ضوورورية فووان‬
‫باستطاعته تيمين كل ذلك أو يرغب باإلشارة إليه في مذكرة اصة منفصلة ترف بالعرض ‪.‬‬
‫‪ )7‬وفي حالة الب الجامعة) من المتع ة المشارة عليه أن يرف البيانات التالية بعوة تصوةي موا يلوزم مون‬
‫قبل الج ات المختصة‪-:‬‬
‫‪ ‬بوورة المتع ووة وكفاءتووه والوكوواالت التووي عنووةه إن وجووةت وغيوور ذلووك موون التفاصوويل والمعلومووات ذات‬
‫العدقة التي من شفن ا أن تقنع الجامعة) بقةرته علوى أداء التزاماتوه ب وذا العطواء وتنفيوذها علوى أفيول‬
‫وجه‪.‬‬
‫‪ ‬عنوووان المتع ووة الكاموول الرسوومي فووي غووزة أو ووارج غووزة أمووا إذا كووان مقوور المشووترة أو مكووان إقامتووه‬
‫الرسمي ارج غزة فان عليه أن يحةد عنوانا ً له في غوزة يعتبور عنوانوه الرسومي الوذي توجوه إليوه كافوة‬
‫الرسائل واإلشعارات وكل إشعار أو رسالة تبعث مبجلة على هذا العنوان تعتبر أن ا سلم رسميا ً إليوه‬
‫‪.‬‬
‫‪ ‬على المتع ة تقةيم أي معلومات أو بيانات أ رى وإرفاق ا بعطائه إذا ورد ل وا ذكور فوي الشوروا العاموة‬
‫أو الخاصة أو المواصفات أو غيرها من ويائ العطاء‪ ،‬ولصاحب العمل الح في إهمواة أي عورض ال‬
‫يشمل هذه اأمور‪.‬‬
‫‪ )9‬علووى المتع ووة الفووائز بالع طوواء أن يتقووةم إلووى صوواحب العموول وودة فتوورة أسووبوا موون توواريخ إبدغووه باحالووة‬
‫العطاء عليه أو تلزيمه له لتوقيع العقة وعلى المتع ة أن يقةم إلى الجامعة) كفالة حبون اأداء قبول توقيوع‬
‫نموووذج االتفاقيووة وحبووب نموووذج الكفالووة أو التووفمين المرف و وتكووون قيمووة هووذه الكفالووة الصووادرة موون أح وة‬
‫المصارف المبجلة في البلة بالمبلغ المحةد ‪ )%12‬من إجمالي قيمة‬

‫العقة وذلك ضمانا ً لتنفيذ العقة تنفيذا ً تاما ً ولةفع ما قة يترتب على المتع ة وفاء أغراض العقوة وإذا توف ر‬
‫المشترة أو استنكإ أو رفض توقيع االتفاقية أو عجوز عون تقوةيم الكفالوة المطلوبوة لحبون التنفيوذ فعنوةها‬
‫يح للجامعة) مصادرة قيمة التفمين الذي أرفقه بعرضه دون الرجوا إلوى القيواء وال يكوون للمشوترة‬
‫أي ح في المطالبة به أو بفي تعويض بشفنه‪.‬‬
‫‪ )12‬إذا استنكإ الفائز بالعطاء عن تنفيذ العطاء أو تف ر في تقةيم اللوازم كل وا أو أي جوزء من وا عون الموعوة‬
‫المحووةد لووذلك أو ووالإ أي شوورا موون شووروا العطوواء أو يب و أن المتع ووة قووةم للجامعووة لوووازم ال تتف و‬
‫ومواصفات العطاء‪ ،‬فعنةها للجامعة اتخاذ أي من اإلجراءات التالية أو جميع ا معا وذلك دون حاجة إلوى‬
‫إ طار أو إنذار‪.‬‬
‫‪ ‬مصادرة التفمينات المقةمة من المتع ة‪.‬‬
‫‪ ‬شراء اللوازم من اأسوا باأسعار البائةة بالطر يقة والتكلفة اللتين تراهموا الجامعوة مناسوبتين وتيومين‬
‫المتع ة فر البعر و التكاليإ‪.‬‬
‫‪ ‬إحالة العطاء على المتع وة الوذي كوان قوة قوةم سوعرا أعلوى منوه فوي العطواء إذا قبول ذلوك وتحميول المتع وة‬
‫المبتنكإ الفر في البعر أو التكاليإ باإلضافة إلوى إلوزام المبوتنكإ بتعوويض الجامعوة عون أي عطول‬
‫وضر لح ب ا نتيجة الستنكافه‪.‬‬
‫‪ ‬حرمان المتع ة المبتنكإ من االشتراة في مناقصات الجامعة‪.‬‬
‫‪ )11‬إذا كان هناة أي إلتباس أو تناقض في ويائ العطاء أو كان هناة أي حاجة الستفبار عن أي توضيح‬
‫في ويائ العطاء فعلى المتع ة أن يتقةم بطلب طي لدستفبار مون أجول توضويح أو إزالوة االلتبواس أو‬
‫التناقض في موعة يبب التاريخ المحةد لفتح العطاء بما ال يقل عن مبة أيام ) ويقوم صواحب العمول‬
‫بتوزيع جواب االستفبار على كافة المشتركين المتقةمين للعطاء وال يجوز أن يتخوذ ماول هوذا التوضويح‬
‫مبررا ً لطلب تمةية الموعة المحةد لتقةيم العروض‬
‫‪ )12‬إذا وجة أي من العروض المقةمة نقصا ً أو طف أو تناقيا ً بين حباب جملة أي مبلغ وما يجب أن تكون‬
‫عليه هذه الجملة بتطبي البعر الموضوا أي بنة في جوةوة الكميوات فللجامعوة الحو فوي تعوةيل جملوة‬
‫المبلغ بما يتفو وتطبيو البوعر الموضووا علوى الكميوة الموضووعة لوذلك البنوة وبالتوالي تعوةيل حاصول‬
‫اليرب أو مجموا البعر أو المبلغ المقةم للعطاء وإذا حةث ودف بوين العوةد الموذكور باأرقوام كتابوة‬
‫بالحروف فتعتبر الكتابة بالحروف هي الملزمة وكذلك إذا حةث طف في جموع قيموة مختلوإ البنوود فوان‬
‫الجامعووة ل ووا الح و فووي تصووحيح المجموووا ويكووون المجموووا المصووحح فووي ماوول هووذا الحوواة ه وو الملووزم‬
‫للمتع ة‪.‬‬
‫‪ )13‬تحتفظ الجامعوة) بحق وا فوي إهمواة أي عورض غيور متقيوة بكول موا ورد فوي هوذه التعليموات كموا تحوتفظ‬
‫بحق ا في رفض أي عرض دون بيان اأسباب ويح لوه ا تيوار العورض الوذي توراه مناسوبا ً ل وا وإحالوة‬
‫العطاء دون التقية بفقل العروض قيمة ويكون كل ذلك دون أن يكون أي متع ة لم يفز بالعطاء أي حو‬
‫في مطالبة الجامعة) بفي تعويض‪.‬‬
‫‪ )14‬يح للجامعة) إلغاء جميع العروض المقةمة من المتع ةين وإعادة ارح الويائ مرة يانيوة إموا بشوكل ا‬
‫الحالي أو بعة إجراء أي تعةيل عليه وفي هوذه الحالوة يحو لجميوع المتع وةين الوذين اشوتروا العطواء فوي‬
‫الموورة أو الم ورات البووابقة مقابوول الووامن أن يحصوولوا فووي الموورة أو الموورات القادمووة التاليووة علووى الويووائ‬
‫المطروحة مجةدا ً وكاملة بةون مقابل‪.‬‬

The electrodes are protected by "cassettes" and are easily removed for cleaning

The slit lamp is a low to medium
power binocular microscope
mounted to be used horizontally
Illumination is provided by a
projected light which can be large
and circular or narrowed down to
a slit-like beam.
The large wide beam can be
used to survey all or most of the
specimen (whole globe or corneal
button).
The beam is narrowed to give a
beam as wide as the cornea is
thick to form a parallelepiped
volume (a box of illuminated
tissue).
The depth, width and position of
small abnormalities can be
evaluated with this.
The beam can be further narrowed to form an optical section (so thin it's just
discernible). This allows the smallest change in clarity to be evaluated, as well as
pinpointing the depth of pathology. A short parallelepiped or the smallest circle
projectable is used to perform specular reflection, the technique in which the
endothelium is visualized.
The light source usually has several or continuous levels of illumination.
Medium to high illuminations are used for most purposes. High should be used
in optical section and specular reflection. Several levels of magnification are
usually available as well. Low power (~10x) is used for survey, medium to high
(16-40x) for optic section and parallelepiped, and high (40x) for specular
reflection. Normally the light is focused at the same point as the microscope.
This configuration is described as parfocal. If it is not, the beam has been
decentered or the oculars are not adjusted to their neutral position. Both
situations are easily remedied. Check the instrument's manual for how to rotate
the slit projector beam.
The slit lamp is mounted on a moveable stand.
A joystick is used to move it right and left, as well as back and forth (focusing).
The joystick or similar arrangement is used to raise or lower the slit lamp. The
slit projector pivots around the microscope mount and can be swung 180
degrees in front of the specimen. The beam can be projected tangential,
perpendicular, or at any angle to the specimen. While performing an exam the
light should be moved frequently, as the different angles at which the light hits
the cornea may make pathology more obvious. Direct illumination (the beam
directly pointed at the specimen) shows gross pathology, while
retroillumination (the beam decentered to illuminate behind the area of
interest while it is still in focus) may bring out subtle optical changes such as
thin vascularization (blood vessels), small incisions, and endothelial
abnormalities. Sclerotic scatter uses a beam ~1mm wide, half the height of
the cornea, pointed at the limbus, to view subtle abnormalities. The light
spreads by total internal reflection through the cornea and scatters off of any
pathology.
