Elyria City Health District -Plan Submittal Requirements for Tattoo, Piercing and Permanent Cosmetic Operations Name, address and Phone number of operation owner Name, address and Phone number of building owner (if not same) Location: Must be commercially zoned – not permitted as household business Operational: (a floor plan to be submitted) Floors, walls must be impervious, cleanable (indicate materials made of) Premises must be at least 100sq ft (indicate square footage) Each room to be separated from waiting patrons by panel or door, provision to be made for complete privacy Minimum 40 ft candle of lighting Tables and other equipment must be easily cleanable (indicate materials made of) Restroom facilities must be available Hand sink must be in close proximity to each operator Forms to be developed and submitted: Procedures on how to disinfect and sterilize non-disposable items Log of all tests performed on sterilizers Log of date, time and name of person or independent testing facility performing tests Procedure for remedial action in event of failed test Procedure to ensure general health & safety of employees Written post procedure care instructions Reporting form to be submitted to board in event of reported infection Trainings (copies of certificates to be submitted): Basic first aid Bloodborne pathogens Hepatitis B vaccination Certificates or statements indicating experience/apprenticeship in procedures Equipment (make and model numbers to be submitted): Ultrasonic machine available to clean non disposable instruments Steam autoclave – 15minutes @ 15lbs per inch/2500F Note: for an artist or apprentice to apply tattoo or piercings they must provide proof that they have received at least 2 of the 3 required doses of hepatitis B vaccinations and show proof that the 3ed dose is scheduled The items listed above do not represent all of the operating requirements, just those items needed for plan review. A copy of the complete tattoo rules of the State of Ohio can be obtained on the Ohio Department of Health Website. The City of Elyria has additional requirements; those can be found in the Elyria Codified Ordinances. As all proposed operations must be reviewed by the Board of Health, it is strongly recommended that all information be provided to ECHD no later than the 20th day of the month to allow us time for plan review and presentation to the Board in the following month. The Board of Health meets on the 2nd Monday of each month. You will be notified in writing of the Board’s decision – an inspection in required before authorization will be given – the license fee is $100 per year (non prorated) – the license year runs from Jan 1 to Dec 31 – renewal applications will be mailed in December. Potential operators are reminded to contact both the Elyria Building Department and the Elyria Fire Department for their plan submittal requirements.
