This form must be completed; payment included, and handed to the photographer on retake day. The only exception to this would be if you (a) were absent on the original photo day and have not yet had your photo taken or (b) you have not yet had your photo taken and do not wish to purchase a photo package but would like a student ID card and want your photo to appear in the yearbook.
