Social Services Director Highline Care Center, Seattle 1999-2000 Responsibilities included: Case management, very detailed documentation, coordinate care conference and coordination of resident care with other team members such as doctors, nurses, physical therapist, occupational therapist, dietician and psychiatrist, 1 monitor the resident’s psychosocial well being, serve as resident advocate, promote resident rights, address quality of life issues, assess mood, behavior, participate in psychotropic medication reviews, provide staff in-services, and provide mental status exams, intake services and discharge planning. Social Services Director Monarch Care Center (psychiatric facility), Seattle 1998-2000 Responsibilities included: Case management for challenging population with a number of behavioral needs, which required intervention with aggressive medically non-compliant clients. Provide detailed documentation, care plans and goal development, coordinate care conference and coordination of resident care with other team members such as doctors, nurses, physical therapist, occupational therapist, dietician and psychiatrist, monitor the resident’s psychosocial well being, serve as resident advocate, promote resident rights, assess mood, behavior, participate in psychotropic medication reviews, provide staff in-services, and provide mental status exams, intake services, admission and discharge planning. Provide behavioral and crisis intervention. Assisted with the implementation of the Eden Program. This program promoted a more natural, home like living environment with the inclusion of plants and animals (dogs, cats, rabbits, birds). Promoted quality of life and painted large murals to beautify and create a home like atmosphere. At this facility there were residents who were able to participate in painting murals with me. Activity Director Certified/ Recreational Therapist Cottesmore of Life Care 1997-98 Responsibilities included: Implementation and creation of group and individualized activity programs, provided specialized programming for Alzheimer Ward, intergenerational programs for seniors and day care children. Included: art groups, exercise groups, sensory stimulation, games, music groups and spiritual groups. Coordinate and provide community outings and encourage community involvement in programming. Includes coordinated community plays, carnivals and special events. Activity Director Certified/ Recreational Therapist Jefferson House Care Center (psychiatric facility) 1993-1997 Responsibilities included: provide recreational therapy groups designed to promote positive interactions and behaviors, included traditional activity groups such as art, games and exercise also unique to this facility managed a resident store, implemented work programs for residents, including participation in stocking and managing the resident store, at this facility participated in fund raising and community out reach, supervised volunteer program, and consistently took residents out into the community. The majority of the population carried a diagnosis of schizophrenia. Provided behavioral interventions, crisis intervention for aggressive clients. This was an old cinder block building with very long narrow hall ways which could not be decorated with pictures or paintings because the clientele would remove them. I painted extensive murals throughout the hallways and main rooms. Crisis Line Answerer/ Help Line/ Parent Warm Line Comprehensive Mental Health 1990-92 2 Responsibilities included; Provide crisis counseling over the phone, make assesments. Included suicide intervention, coordination of community services and resources, coordination of mental health services as needed. PRACTICA EXPERIENCE Psychology Intern ( Master level) 2001-2002 Good Samaritan Mental Health Child and Family Services, Puyallup Responsibilities included: provide counseling services to individuals and families at the clinic and through community out reach, (homes and schools), co-led groups. Conducted play therapy with young clients 2-10 years of age, and provided individual therapy services to adults and families, conducted intake assessments, developed treatment plans, goals and maintained the required documentation. Psychology Intern (Doctoral level) 2005-06 Fircrest, Shoreline Responsibilities included provided mental health services to the developmentally disabled population. Fircrest is a residential facility for adults with developmental disabilities. Primarily behavioral interventions as my clients were either verbally limited or non-verbal and most carried a diagnosis of severe to profound mental retardation. Participated in goal setting, plan development, advocacy, care needs assessment with team of providers (psychologist, psychiatrist, medical doctor, nurses, vocational rehabilitation, residential case managers and aides). Participated in individual therapy and group therapy. Participated in vocational rehabilitation and recreational activities with clients and community outings. Developed art therapy /vocational rehabilitation group in which we painted sweatshirts at the resident operated thrift store and painted murals. Provide intake, triage, and individual therapy and facilitate group therapy. Provide psychological assessment, as well as treatment planning and make recommendations for treatment, identify risks. Assessments completed thus far include trauma assessments, risk assessments, high needs evaluation, risk to re-offend, and differential diagnosis, extended family visits. Increasing understanding of the role of the psychologist in a 3 rehabilitative health care setting. Duties: Participate in interdisciplinary pain rehabilitation program evaluations, interviews; participate in psychoeducational/psychotherapeutic group sessions. Increase understanding of the nature of chronic pain and pain management. Learn/gain experience in differential diagnosis in chronic pain patient population with an emphasis on recognizing the presence of mod disorders secondary to chronic pain, the most common being anxiety and depression disorders.
