CLEARWATER Council of Governments JOB POSTING TRANSITION SPECIALIST Summary of Duties: This position is a professional position responsible for working with individuals with developmental disabilities (age 14 and up) and their families to prepare for the transition from school to employment. Duties include: Counseling eligible individuals with disabilities toward employment in mutually agreed upon vocational goals based on an assessment of interest, values, capabilities, and barriers. Working with county boards of DD, schools, and providers to identify potential referrals for services. Working as a team with regional Bridges to Transition Counselor, support staff, vocational rehabilitation Supervisor, and Bridges Program Manager. Meeting regularly with other Bridges To Transition team members to review caseload activity and ensure timely case progression. Working together with Bridges Counselor to conduct vocational rehabilitation referral education activities. Attending Individual Education Plan (IEP) meetings for prospective applicants (age 14 and up). Scheduling intake appointments and conducts intake interviews. Gathering information required for application screens; facilitating completion of application for services and uploading into system. Qualifications Bachelor’s Degree in a human services area (e.g. rehabilitation, counseling, social work, psychology, sociology, special education, communication disorders, rehabilitation teaching) OR a bachelor’s degree from an accredited university in another area AND 24 months experience in the delivery of vocational rehabilitation services (e.g. job development, benefits analysis, certified vocational evaluator, vocational specialist Minimum qualifications must be met to be considered for position and college transcripts must be provided by the applicant. Notes: Office location of the Transistion Specialist is flexible but frequent and regular travel is required to Wyandot County, Seneca County, and Crawford County.
