For a student who has made payment for tuition fees, the student must raise the refund themselves (b) A database of recipients verified by the Head of Student Services will be forwarded to Financial Services for reference and records. (a) The balance amount will be paid by Shell to the student as living allowance (b) The total amount shall not be more than RM12, 500 3.1.4.2 Other expenses are to be borne by the student and successful applicants are not bonded to work in Shell after completion of the course 3.1.4.3 Criteria: (a) STPM/A’ Level results with at least 2A’s and 2B’s (b) CSM Foundation results with CWA 80% and above (c) Accepted to Curtin first year Engineering degree (d) No siblings currently holding Shell Scholarship and not holding any scholarships or loans including PTPTN (e) For Sarawakian and Sabahan students (f) Not more than 23 years old (g) Excel in academic, co-curriculum activities and able to interact and communicate with wide range of people 3.1.4.4 Procedures (a) Create a database with the following categories: i. applicant’s reference number ii. This is to monitor those students who had paid the tuition fees prior to the offer (c) The student must fill in the Refund Request form and forward to the Cashier OFFICE 3.1.5 OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 15 / REGISTRAR WESTERN AUSTRALIAN GOVERNMENT SCHOLARSHIP 3.1.5.1 The scholarship is only for CSM’s existing students who will be going for their the final year 3.1.5.2 The scholarship includes return airfares, traveling expenses, on-campus accommodation sponsored by the Western Australia government 3.1.5.3 Curtin Bentley, Perth will grant the tuition fees for one year, the Overseas Student Health Coverage, the visa as well as the medical report fee, which will is first paid by the students and subsequently to be refunded upon arrival in Curtin Perth 3.1.5.4 Other expenses such as textbooks, living expenses will be borne by the students themselves 3.1.5.5 The Scholarship is only offered to two Business students, two Engineering students and the final candidate from any school 3.1.5.6 The agreement between Curtin and the Western Australian government have been extended to 2009 till further notice 3.1.5.7 Criteria: (a) Malaysian citizen (b) At least CWA of 80% 3.1.5.8 Procedures (a) Create a database with the following categories: i. applicant’s reference number ii. Academic Registrar (c) Arrange for Scholarship Selection Panel to interview short listed candidates (d) Offer letters, Acceptance of Offer, Visa and Overseas Student Health Coverage forms will be issued by Curtin Perth (e) The next best candidate will be selected if the offer is rejected (f) The decision of the Panel is final 3.1.5.9 Conditions (a) Recipients will be briefed in person on the detailed conditions of the award (b) Offer letters can be signed immediately or be taken home for review by students but must be returned to Student Affairs on date stated on the letter (c) Submitted documents will be faxed to Perth, while a copy will be made for reference before the originals will be sent to Perth (d) Students will then be advised to apply for their visas and to proceed with their medical check through the University’s appointed panel of doctors (e) Students must inform the Enrolment Office of the units they plan to enroll in for the coming semester (f) Reports of the medical check and the plan units to be enrolled in will be sent to Perth (g) Loans & Scholarships Office to notify Housing Officer in Perth, Andrew Lee; and Airport Reception Services, Ian Farnie of students’ confirmed date of arrival (h) Loans & Scholarships Office will issue a memo to notify Financial Services that CSM will pay the tuition fee as well as the visa on behalf of Curtin Perth (i) Loans & Scholarships Office will book a return air-ticket (Economy class) for the students either from Miri-KL-Perth or from their hometown and departure date will normally be a few days before orientation in Perth OFFICE OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 17 / REGISTRAR (j) A press release and pre-departure briefing will be held for the student 2 weeks before departure 3.2 EXTERNAL SCHOLARSHIPS 3.2.1 TUNKU ABDUL RAHMAN SCHOLARSHIP (YAYASAN BIASISWA SARAWAK TUNKU ABDUL RAHMAN) 3.2.1.1 This application is for both newly enrolled student to degree as well as CSM’s Foundation students going for Degree courses 3.2.1.2 The Scholarship is offered once a year and the notification of the application and datelines will be 31st July each year 3.2.1.3 The Scholarship covers living allowances, tuition fees, air-ticket (economy class) and other allowances 3.2.1.4 Application forms can be obtained from Student Services or downloaded from Yayasan’s website. Students are to submit the completed application forms with the required documents directly to Yayasan 3.2.1.5 Short listed candidates will be notified directly by Yayasan 3.2.1.6 Successful applicants are required to provide a copy of the agreement to Loans & Scholarships Office 3.2.1.5 Criteria: (a) Grade B or above for STPM (b) CGPA 3.00 (74%) and above for Diploma or Foundation results (c) For Sarawakian with Malaysian birth certificates (d) Full time enrolled Degree, Masters or Doctorate students only in ECommerce, Electronic and Communication and Mechanical Engineering courses (e) Students must achieve a CGPA of at least 2:00 every semester 3.2.1.6 Payment process: (a) Financial Services will invoice YBTAR for tuition fees payment each semester (b) Invoice will be based on units enrolled/credit hours taken OFFICE OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 18 / REGISTRAR (c) Students who withdraw/on leave of absence/terminated must notify YBTAR in writing (d) A student who fail a unit and repeat that same unit in another semester will not be funded by YBTAR 3.2.2 JABATAN PERKHIDMATAN AWAM (JPA – SABAH SCHOLARSHIP) 3.2.2.1 This application is for full time enrolled undergraduates in Computer Science, Engineering, Accounting and Tourism Management from Sabah and Labuan only 3.2.2.2 The Scholarship is offered once a year and covers living allowances of RM3, 300, tuition fees, return air-ticket (economy class) and textbook allowance of RM200 3.2.2.3 The students will be sponsored every year with the condition that the students need to perform well each semester 3.2.2.4 Summer Course is covered under the Scholarship 3.2.2.5 The applications begin in March and close in April and are advertised by JPA in the Sabah newspapers 3.2.2.6 Application forms may be obtained directly from JPA Sabah, district offices, or municipal councils at RM1 each 3.2.2.7 Criteria: (a) Distinction in 5 SPM subjects including Bahasa Malaysia and other subjects related to the major (b) Good grades in four STPM papers including General Paper 3.2.2.8 Students are to submit the completed application forms with the required documents directly to JPA and short listed candidates will be notified by JPA 3.2.2.9 Successful applicants are required to provide a copy of the agreement to Loans & Scholarships Office 3.2.2.10 Students must maintain CGPA of 2.00 and above, and submit their semester results to JPA OFFICE OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 19 / REGISTRAR 3.2.2.11 Students who wish to change course other than the stated course in the agreement must inform JPA to request for permission as well as a letter to confirm approval of course transfer 3.2.2.12 Students who wish to extend the duration of sponsorship must request for permission from JPA. PTPTN forms are available from the Loans & Scholarships Office and the cost of RM 6:00 which had to pay at Cashier Counter. There are two submissions in a semester which is 15th April and 15th May for 1st semester where as 15th August and 15th Sept for 2nd semester. However we will advertise earlier in order to be able to check through all the forms. All application forms are to be submitted to Loans & Scholarships Office. The due date to submit forms to Loans & Scholarships for Semester 1 will be on the 30th March and 28th April and Semester 2 will be the 28th July and 28th August every year. Upon receiving the application and other documents, we must check to make sure the documents given are completed and forms are properly filled. To make sure that the IC/names/course code & name/ID/income tax number of parents/date of commencement of course/details of SPM result/qualification entry to degree/salary of parent (must fill in net amount). Create a database on list of student with details such as IC/ID/Course enrolled/ names/Gender/BCB account number/telephone number/min. entry requirement. This name list in database should be group according to course enrolled. The documents must be send to PTPTN at least 5 days before submission due date to avoid institution marks being deducted which will affect our grades. All forms and other documents must be photocopy before being sent out. Documents send out are: OMR Application form (attached a cover letter and list of students according to course they are in and have to be put on top of all application.) 1 copy of JPS letter for course approval. 1 copy of PTPTN Course code PTPTN submission due date. Individual application send not through the institution will not be entertained. Upon received of the documents, PTPTN will inform in writing on the status of application as of; I. Completed documents – will be brought up for committee meeting and board of Directors meeting for approval. II. Not completed - will not be considered. III. Not qualified applicants – will not be considered. This might be due to no course approval from JPS/still owing PTPTN or other reason stated by PTPTN 3.5.1.4 APPROVAL PROCESS For normal application send by institution, PTPTN will take approximately 21 days for the loan to be approved. Arus Perdana can only be when the institution can get about 150 student to apply for PTPTN then we can invite PTPTN to come over to Curtin to give instant checking and approval and at the own expenses otherwise if lower than 150, we will have to bear the expenses. Upon approval by board of Directors, PTPTN will send out the offer letter and agreement to Institution and together with the list of names of those being offered. PTPTN had given a schedule timetable on submission of students’ agreement for Curtin University .Refer table as below. Upon received of the agreement, we will have to arrange for a date for the students to come for signing of the agreement. Loans & Scholarships must make sure to return the agreement before due date given in the PTPTN cover letter failing which will affect our institution grade. Before filling in the offer letter and agreement, the student must check through their the name/address/course program/bank account number/IC are correct. If the bank account number/address and IC are not correct, student will have to fill a form on changes of these particulars. See attachment on sample of form. Student must fill the agreement carefully and must not make mistake. All forms must be filled in with black/blue ball point pen and must be capital letters. Any mistake made, the student must make a straight line across the word/sentence and sign at the side of the word/sentence. Students are not allowed to use blanco/correction pen. Copy for PTPTN and Surat Kebenaran 1, 2, 3 will be return to PTPTN. In the offer letter student will need to fill in their names/ course offered/ agreement date/account number/bank name. Refer attachment on how to fill offer letter and agreement. Students will need to pay at cashier counter RM 45.00 to buy LHDN stamp. RM 40.00 for buying LHDN stamps and RM 5: 00 as service charge. The stamps can be bought from Post Office. 2 copies of student agreement have to go for stamping at LHDN. Before proceeding for stamping, a memo must be sent to Financial Services, attention to Ms Sara Wong to request for cash payment. The memo should be done as soon as all students are done with the signing of agreement. This is to avoid delaying of time due to the process of cash payment from the financial services which will affect our due date to return the documents. Payment will starts effective from the date of their offer letter and agreement and first disbursement will be on that current semester their loan approved. Maximum amount will be RM16,000 per year for Commerce and Arts degree and RM16,500 per year for Engineering and Science degree The loan only covers for the tuition fee only not other expenses 3.5.1.5 PAYMENT PROCESS 1st disbursement of payment to the student are done only after the agreement is successfully completed by student and accepted by PTPTN. For the following semester’s payment, it will be made after the confirmation status is received from the institution (Curtin). The following status are as follows; I. Active and is still studying II. Pass previous semester with CGPA 2:00 and above III. Does not hold any other loan/scholarship IV. Any other status stated from time to time Every semester PTPTN will send a diskette with list of students under PTPTN loan for confirmation of their status. Upon receiving of the diskette (hardcopy) on the student list, Loans & Scholarships will have to get copy of the student’s transcript from the examination department. OFFICE OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 40 / REGISTRAR As required by PTPTN, the results will then be calculated to get the CGPA of the student. Once the CGPA are obtained, then we can proceed to change the status of the student according to the code that had been given by PTPTN. (see attachment) Loans & Scholarships will deduct RM 100.00 from payment amount in the listing and this will acknowledge PTPTN to direct credit the RM 100.00 to students account as notification for students and that the balance of PTPTN payment is paid direct to Curtin account. Upon received of the payment from PTPTN, PTPTN will issue a list of names of student under each payment voucher. We have to contra check all the names in order to make sure that no students are left out for payment. Nevertheless refund to the student can only be done after we received listing of student names from Bank Bumiputra as this listing is the final listing on successful transaction. PTPTN had given a schedule for confirmation status and payment. All students who have been offered the PTPTN loan are requested to have an insurance . This is to ensure that the repayment of the said loan be continued by the insurance, should the student passed away or permanent disablement. PTPTN will decide on the insurance cover and payment premium will be deducted from the loan amount. Payment to student who go for leave of absence or postpone his course will be on hold upon reasons given by the institution. If a student goes for leave of absence/postponed his course after payment from PTPTN then the next payment will be on hold by PTPTN. PTPTN will hold any payment made to student if the exam result for previous semester is less than GPA 2.00. There is no postponement of payment to any student on any payment withhold from them. Student who transfer/withdrawn from an institution because the course had been stopped are allowed to appeal for continuation of loan but the level and course offered must be the same. Documents needed to forward to PTPTN are appeal letter from student that states reason for transfer of course, release letter from previous institution and acceptance of offer from the new institution. A student who was offered PTPTN loan in his previous institution for his previous course but has withdrawn/terminated can only apply for the loan after he settled the previous loan amount. A student taking PTPTN loan in his Diploma course and has successfully completed the course can apply for second application to PTPTN in degree course unless second application of PTPTN loan is of the same course which are not allowed by PTPTN. Appeal for extension on PTPTN loan will not be considered unless for reasons such as changes in course structure and health problem of a student. PTPTN loan will not cover for summer course. OFFICE OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 42 / REGISTRAR 3.5.1.6 REPAYMENT PROCESS PTPTN will charge a 3% interest on loan and administration cost and every year based on balance of loan each month. Repayment for PTPTN loan will start six (6) months after graduation or after a student has secured a job, whichever is earlier. Payments can be made through Lembaga Hasil Dalam Negeri(LHDN), Maybank2U, Bumiputera Commerce Bank, Bank Islam or Post Office. Payment made to PTPTN must be by Bank Draft and paid to Ketua Eksekutif, Perbadanan Tabung Pendidikan Tinggi Nasional. A student can pay PTPTN the whole amount upon graduation, or any amount but not less than the amount stated by PTPTN in their agreement. If a student settles the whole amount within six months after graduation, there will be no interest incurred. A student who appeal for deferment of payment or rescheduling for reasons such as extension of duration of course/have not completed studies can write a letter to PTPTN. The student must request a confirmation letter from the student services department on his status. 3.5.2 YAYASAN SARAWAK LOAN 3.5.2.1 Application received with all the relevant documents: Sarawakian only Identity card of Student Students need to show the evidence of rejection (i.e a letter from PTPTN) that they did not get the offer from PTPTN/get a support letter from head of student services stating the student condition. Full-time enrolled Foundation and Degree student of CSM BM in SPM must be credit Testimonial of the evidence of extra-curricular activities and socio-economics Offer letter from Curtin Not holding any scholarships or loans The closing date is ongoing each year. The form can be obtained from Loans & Scholarships or download from the Yayasan website, Resident Office or by mail with the self-addressed envelope of RM1 for Yayasan staff to post the form to the student later. OFFICE OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 43 / REGISTRAR The form must be sent together with all relevant documents and must be certify by class A government officials /Registrar/Head of Student Services. 3.5.2.2 The student need to submit the documents together with the application form to Yayasan directly as per mentioned in the application form. 3.5.2.3 Approval of application depends on the number of student apply for that particular year. 3.5.2.4 There is no conversion of loan to scholarship but this depends on the offer and agreement made between Yayasan and the student. 3.5.2.5 Beginning of a semester, Loans & Scholarships will send student transcript to Yayasan. 3.5.2.6 Payment to student will depend on the CGPA obtained. Please refer as follows; 2:00 pass and with payment. 1:8 – 1:9 Yayasan will pay but with a reminder letter 1:00 – 1:79 – Yayasan will pay but the student will have to produce a show course letter. 1:00 – No payment at all unless there is improvement in the student performances the next semester. 3.5.2.7 Payment of tuition fees are paid to Curtin account instead of student account. The students will need to : Fill in the letter of consent from us as this will help us to monitor their status of payment in future We will help Yayasan to monitor all the students’ their semester results Assist Financial services to monitor their payments Assist the students once the agreements came to us and we will a copy of the agreements especially the details of the loans for each individual students OFFICE 3.5.3 OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 44 / REGISTRAR SARAWAK HIGHER EDUCATION FUND CHARITABLE FUND CHARITABLE TRUST (YAYASAN SARAWAK HI-ED STUDY LOAN) 3.5.3.1 Application received with all the relevant documents: Sarawakian only Full-time enrolled Foundation and Degree Engineering student of CSM BM in SPM must be credit Testimonial of the evidence of extra-curricular activities and socioeconomics Offer letter from Curtin Not holding any scholarships or loans The closing date is ongoing. It will be advertised in the local newspaper from time to time. (e.g Borneo Post). Loans & Scholarships will also do the pop-up notice and notices around the campus to notify the students to do the submission of form to us. The form can also be obtained from Loans & Scholarships counter or download from the Yayasan website The student needs to submit the documents together with the application form to us directly before the due date. 3.5.3.2 Once approved, Yayasan Hi-Ed will send us the statement of the amount each student received together with the agreement which the student needs to sign before the loan actual took effect and there is a dateline for submission which they will inform us when we received the agreements. 3.5.3.3 If agreement received a) Agreement needs to be signed We will arrange a date, time and venue to group the students to brief them on the step-by-step to sign the agreement The students will need to : Fill in the agreement properly Signature of sureties usually should be the parents, otherwise if the parents are not working or deceased then, another closed relative will be OFFICE OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 45 / REGISTRAR the sureties as they will act as a guarantor if the students failed to pay in future. Both the sureties need to provide photocopies of ICs and their salary slips. The letter of offer should be signed and sent back a copy to Yayasan, and another copy will be kept by the students. 4 sets of agreements should be registered at the Income Tax Department for stamping by the students ( 2 set of agreements are for the sureties, 2 set for Yayasan and another one for student reference and safekeeping). However, all four set of agreements need to send back to Yayasan Form 3 (Reply Form) need to be filled and signed by the student and faxed to Yayasan Form 4 (Surety Form) also need to be filled by the sureties and gave to us o Form 5 (Letter of Authorization) must be filled and signed by both the student and a witness and gave to us We will send the agreement together with Form 4 and 5 to Yayasan before the due date 3.5.3.4 If loan approved a) Payment will be made: Curtin will receive the money The students will need to : Check with us, better with the Financial Services side on the fund Refund will only be made if the students already pay the money and the fill in the refund form The students still enroll and study with us OFFICE 3.5.4 OF THE Issue : 1 /Loans & Scholarships Date : 06/03/2016 Page : 46 / REGISTRAR BAKUN CHARITABLE TRUST 3.5.4.1 Application received with all the relevant documents: Student resided in Belaga only Full-time enrolled Foundation and Degree student of CSM Academic transcripts for SPM/STPM and the Foundation/Matriculation need to be submitted together Birth certificates of the applicants as well as the parents’ IC of both the applicants and the parents Testimonial of the evidence of extra-curricular activities and socio-economics Offer letter from Curtin Not holding any scholarships or loans Sponsored for one year only of RM3,000 only The scholarship offered once a year in April and closed in July. The form can be obtained from our counter here. 3.5.4.2 The student need to submit the documents together with the application form to Tabung Amanah Bakun directly as per mentioned in the application form 3.5.4.3 Once, been selected it is the responsibilities of the student to pay the tuition fee partly with the money given by them. 3.5.5 EPF WITHDRAWAL SCHEME FOR EDUCATION 3.5.5.1 Application received with all the relevant documents: EPF members and/ or EPF member’s children Full-time enrolled students of CSM Offer letter from Curtin Letter of confirmation of the course study, duration, course fees and others. If EPF not approved the withdrawal and it is due date for payment, then the student need to pay the tuition fee first and get the refund from us later in order to do enrolment. 3.5.6 THE SOCIAL SECURITY ORGANIZATION (SOSCO) EDUCATION FUND 3.5.6.1 Application received with all the relevant documents with the requirements stated below: Dependent children of an employee receiving invalidity Pension or periodic permanent disablement who are below 21 years of age and unmarried at the time of the application Dependent children of an insured person who has died as a result of an employment injury or while receiving invalidity pension or before reaching age 55 and who fulfils the contribution qualifying conditions. They should be below 21 years of age and unmarried. Dependent children who are above 21 years of age, receiving a monthly benefit and already studying in an institution of higher learning and unmarried at the time of application. Dependent children who have been offered places and have registered in a university of college or Local Institution of Higher Learning including institution that has twinning programme on condition the degree is completed locally. They must be also registered with the Ministry of Education, to acquire a degree, diploma or a certificate which is awarded by the National Vocational Training Council, Ministry of Human Resources. If SOCSO not approved the withdrawal and it is due date for payment, then the student need to pay the tuition fee first and get the refund from us later in order to do enrolment. Therefore, we are in the middle of negotiation and compromise for more courses given to the students. 3.5.8.7 Students will need to pay them back a year after they graduate according to the statement posted to them for repayment. 3.5.8.8 Students are not allowed to extend their courses which vary from the duration mentioned in their agreement. Any extra extension, the students will need to bear the cost by themselves. 3.5.8.9 Summer course will not covered by MARA. If the students apply for Student Exchange to Curtin Perth, MARA might consider the payment since the tuition fee is applicable with Curtin Sarawak but is yet advised by the MARA officer in charge (En. The Orientation kit will be ready for collection once they enrolled at the Admission and Enrolment counter (Counter 3 and 4 at Student Services Department). Duplicates need to be originals Support letter from the University to notify them on the claims Need the support letter from the club advisor or the president to clarify if the injury caused by activities involved in the clubs or societies Need to fax to them to notify them on the claims while sending the originals to them Claims not more than RM1,500 for medical claims unless total disability .
