DOCUMENTATION CHECKLIST
Use this checklist when assembling a documentation file for a normal merit increase or for first-time
requests for no action (Senate Series) or normal merit increase or for reappointment (Non-Senate Series)
for candidates in any of the following:
Senate Series
Non-Senate Series*
Professor
Professor in Residence
Professor of Clinical
Adjunct Professor
Health Sciences Clinical Professor
(First Reappointment for HS)
The Short Form may be used for the following normal merits in the series above:
Assistant Professor I-II; II-III
Associate Professor I-II; II-III
Professor I-II; II-III; III-IV; IV-V; VI-VII
A. Checklist.
B. The original and one copy of the dossier. Each dossier should include:
1. Signed Academic Personnel Action Summary (Form UCI-AP-24).
2. (Non-Senate) Supplemental Information* (Form UCI-AP-137A).
3. Dean’s statement (or acknowledgment of review), if provided.
4. Department chair’s statement (or acknowledgment of review), if provided.
5. Department evaluation. The department or a designated committee should have reviewed the
evaluation (with notation if copy has been provided to the candidate). The evaluation should
contain a report of the faculty opinion and vote and an evaluation of the candidate’s
performance in the following areas:
a. Teaching
b. Research and creative work, if applicable
c. Professional competence and activity
d. University and public service
6. Profile of statistical teaching summaries.
7. Addendum (Form UCI-AP-10; or, UCI-AP-10-Clin) or Review Profile.
8. Curriculum Vitae.
C. Signed Certification Statement (Form UCI-AP-50).
D. One set of individual teaching evaluations. (Optional - As requested by Dean).
E. One copy of all publications or creative activity since last positive review. Number each
publication as it is numbered on the Addendum or Review Profile.
