Name MERIT INCREASE, ACCELERATED MERIT INCREASE, REAPPOINTMENT, NO ACTION, FIVE YEAR REVIEW OR PERFORMANCE REVIEW DOCUMENTATION CHECKLIST Use this checklist when assembling a documentation file for a merit increase; accelerate merit increase, reappointment, no action, five year or performance review, for a candidate in the following: Professional Researcher Series Assistant Researcher Associate Researcher Researcher A. Checklist. B. The original copy of the dossier for normal merit increases. The original dossier and one copy for other actions. Additional copies may be requested if they are needed. Each dossier should include: 1. Signed Academic Personnel Action Summary (Form UCI-AP-22). 2. (Non-Senate) Supplemental Information (Form UCI-AP-137A). 3. Dean’s statement (or acknowledgment of review). 4. Department chair’s statement (or acknowledgment of review), if provided. 5. Department letter. The department, principal investigator, or a designated committee should have reviewed the letter. The letter should contain a report of the faculty opinion and vote and an evaluation of the candidate’s performance in the following areas: a. Research and creative work b. Professional competence and activity c. University and public service 6. Candidate’s response to department Letter, if submitted 7. Identification and qualifications of external referees (Form UCI-AP-11), if provided. 8. Appropriate number of letters of evaluation in chronological order (most recent date first), (with notation if copy has been provided to the candidate) if provided. 9. Candidate’s comments on redacted copies of confidential letters, if submitted. 10. List of evaluators suggested by the candidate and indication of any considered to be inappropriate, with explanation, if provided. 11. Sample(s) of letters of solicitation, if letters are submitted. 12. Professional Research Series Documentation Summary (Form UCI-AP-56). 13. Academic – Career History & Publication Record (Form UCI-AP-99). 14. Curriculum Vitae. C. Signed Certification Statement (Form UCI-AP-50). D. One copy of publications or creative activity since appointment or last promotion. Additional copies may be requested if they are needed.
