Transitional Discharge Modal: Research Project/3 Month Contract /Aug-Oct 2014

To bridge clients with various mood, anxiety, comorbid and concurrent disorders
from the acute inpatient care of the Complex Mood and Anxiety Unit into the
community. Providing hope, building a collaborative therapeutic relationship
while inpatient, transitioning clients into the community and connecting with
outside recovery supports, meetings, etc. in order to ongoing to enhance
recovery and thus quality of life

Individual counseling sessions, facilitation of various group recovery meetings,
such as WRAP (wellness recovery action plan) and other recovery modal
sessions.

Consulting with psychiatric team and social worker with respect to case
management of individual needs and plan of care
