Q. Who can I enroll in my benefits? A. Eligible dependents include: your spouse; unmarried children under the age of 23; your domestic partner; your domestic partner’s unmarried children under the age of 23; unmarried enrolled children or domestic partner’s children, incapable of self-support. FedEx Office Benefits Administration 2010 Frequently Asked Questions Q. I was just rehired with FedEx Office. When am I eligible for benefits? A. If you are rehired within six months of your termination date, and you were previously eligible for benefits, you will be reinstated the first of the month following your rehire date in the same benefit selections you had when you left FedEx Office. If you are rehired after six months of when you terminated, your benefits effective date will be the first of the month following or coinciding with three continuous months of regular, full-time employment Q. Why can’t I add, drop, or change my benefits whenever I want? A. Our benefit plans are governed by Section 125 of the Internal Revenue Code. Under Section 125, IRS stipulations say you cannot add, change, or drop your coverage except at annual enrollment or when you have a qualified change in family status that affects your benefits. A qualified change in family status includes marriage, divorce, birth of child, death, and spouse gains or loses benefits. If you experience a qualified change in family status, you have 31 days from the event to make changes to your benefit elections. If you do not make your changes within 31 days of the event, you must wait until the next annual enrollment to make your changes. Please refer to the Health and Welfare SPD on the fedexoffice.ehr.com website for more information about qualified changes in family status. Q. Can I enroll in just the Dental or Vision Plan? A. Yes, providing you meet the eligibility criteria, you are eligible to enroll in the dental and vision plans. You are not required to be enrolled in the medical plan to be enrolled in the dental or vision plans. You can also enroll in the medical plan without enrolling in the dental or vision plans. Q. How can I find out if my doctor is in-network? A. A “Contacts” tab is located in the benefits library of the fedexoffice.ehr.com website. Here you can access both medical and dental providers. You may also contact Anthem Blue Cross Customer Service at 1.888.252.6941 for assistance with finding a provider. Q. I tried to enroll but my PIN doesn’t work. What do I do? A. Your PIN is your month and day of birth in 4-digit format. For instance, if your birthday is 01/01/1970, your PIN will initially be set as 0101. For your protection, you can also change your PIN at any time. Please contact the FedEx Office Benefits Service Center at 1.866.866.9050, option 0, if you have problems logging in. Educational Assistance FAQs Q. When am I eligible to participate in the Educational Assistance Plan? A. All full time and part time team members are eligible to participate once they have completed three months of continuous employment with FedEx Office. Upon completion of the course(s), submit your approved Educational Assistance Plan Reimbursement Form and all original receipts for tuition and other costs, as well as a copy of your grade report showing a “C” or better to the Plan Administrator. Educational Assistance Plan Reimbursement Form can be found fedexoffice.ehr.com under “Forms”. 401(k) Plan and Employee Stock Purchase Plan FAQs Q. When am I eligible to contribute to the 401(k) Plan? A. Team members can begin contributing to the FedEx Office 401(k) Plan on the first of the month following one month of employment. Deductions will begin with the first full pay period after your effective date. Q. How do I enroll the 401(k) Plan? A. Logon to the fedexoffice.ehr.com website and the 401(k) enrollment website is located in the Benefits Library under Planning for the Future; to enroll, click on the website with the caption “for newly eligible employees” next to it. You can also contact Vanguard directly at 1.800.523.1188 for assistance. Q. When am I eligible to buy stock through the Employee Stock Purchase Plan (ESPP)? A. Providing you are the age of majority in the state in which you reside you may enroll in ESPP . You must be a full-time or permanent part-time employee. Managing directors, officers of the company or members of the board of directors are not eligible to participate in this plan. Q. How do I enroll in the ESPP? A. Enroll in the ESPP by calling Computershare at 1.800.326.6150 or by logging on to Computershare’s website at www.computershare.com/employee/us. You will need your PIN and plan name FDX.
