SEARCH FOR THE OUTSTANDING ALUMNI OF THE YEAR

To recognize the outstanding achievements of the alumni of the University of San Carlos who have significantly
advanced their chosen profession, benefited their fellow Carolinians, their family and the community;
To confer awards in the categories in which they prominently made outstanding achievements; and
To bring these awardees close to the University by actively participating in the Carolinian Alumni Support
Program.
Alumni Semper Fidelis Awards
The Chairman of the Alumni Association’s Board of Directors organizes an Awards Committee who selects from the roster
of nominees those who are for final endorsement to the Administration and approval by the University’s Board of Trustees
through the University President:
Three main categories of the Alumni Semper Fidelis awards are the following

Scientia -

Virtus
3.
Devotio -

Knowledge Award, for exemplary performance and outstanding contribution in education,
academic research, science, literature and creative endeavours
Leadership Award, for demonstrating moral integrity and proficiency in the practice of one’s
profession in business management, civil administration, public service, and entrepreneurship
Service Award, for dedicated service to causes for the underprivileged and community service
exemplifying the Christian values of love, justice, and harmony among peoples
CRITERIA FOR CHOOSING
THE OUTSTANDING ALUMNI AWARDEES
The nominee –
1. Must be a graduate from any college or basic education department of the University of San Carlos;
2. Has exemplified in his daily life the University motto: scientia, virtus, et devotio in following the core values of
integrity, professional excellence in the chosen field, commitment to service for people, social responsibility
especially for the uplift of the underprivileged, and leadership in civic and religious activities to achieve harmony
and peace among people;
3. Submit documents or testimonies regarding the following or which are relevant to the category of award to be
conferred:

positions of prominence in the profession/field of endeavor
achievement in field of work
recognition or awards earned in the local, regional, national and/or international levels
production/ publication notable works, researches, experiments, projects and others
active membership in professional, socio-civic and religious organization and/or charitable
institutions
3.6. leadership or participation in community service projects.
RULES FOR SUBMISSION OF NOMINATION
1.
Any department, college, alumni chapter or alumnus/alumna may nominate candidates for the Most Outstanding
Alumni of the Year Award.
2.
All nominations should be coursed through the Chairs of the Alumni Relations Council of the respective
College/School or Basic Education Departments for submission to the Office of Alumni Affairs, Scholarships and
Job Placement.
3.
A nomination form must be submitted with the following documents:
3.1 Curriculum Vitae, duly signed by the nominee.
3.2 5 pieces 2 X 2 recent & colored picture with formal attire
3.3 pictures during student days at USC, at work, with family & community
3.4 proof of achievements, citations, testimonies and other related-documents
3.5 200-word write-up, in third person, about the nominee

Documents may be submitted to the following respective Chairs of Alumni Relations Councils of the
College/School or Basic Education Departments, who, in turn, will submit such documents to the Office of
Alumni Affairs, Scholarships and Job Placement on or before July 25, 2015 (Saturday):
