† Complete and submit online graduate school application through the Apply Yourself website:

† Submit Application fee payment
Domestic application fee: $45.00
International application fee: $60.00
† Request one (1) official transcript to be mailed from the issuing institution to the UAB Graduate School office at
the following address:

† Request official GRE test score be sent to University of Alabama at Birmingham
International applicants only
† Request TOEFL test score be sent to UAB - UAB Institutional Code: 1856
† Request WES (www.wes.org) transcript evaluation be sent to UAB Request one (1) official transcript
evaluation in the form of a catalog match from World Education Services (www.wes.org) or Educational
Credential Evaluators, Inc.(www.ece.org) to be mailed to the UAB Graduate School (see address below).
† Mail Financial Affidavit of Support to: UAB Graduate School
