 Complete Student Information (1 per family)
 Complete the Health Information Form & Parental Permission Form (1 per child)
 Provide up-to-date Medical Records to the school Secretary before classes begin in
August. This includes:
o Birth Certificate
o Immunization Certificate
o Medical Exam
o Eye Exam
o Parental Permission Form
 Complete Family Referral Form if applicable (this is for new families that were
referred to BCA by a current family)
