sets are not included with ResTel service. However, they can be rented from the IT

Additional Services
Directory Listing includes your name and number on the Telus 411 system and
publishes your listing in the annual Telus White Pages directory distributed in July.
Voice Mail records messages that are retrievable through any phone.
Long Distance Authorization Code enables you to charge long distance calls to your
account. For more information, including long distance rates visit
www.it.ubc.ca/phones/ldrates.
Extension Jacks or changes to jack wiring following the initial installation will cost $25
per jack.
Additional Line can be installed for $30 per line, a $50 deposit, and a $25 per month
Billing and Payment Options
Basic ResTel Service, Voice Mail Service, Directory Service Charges, Long Distance calls
and other one-time charges are billed on the following month in which the charge
occurred.
All applications require a security deposit of $50 which will be applied to your final
statement after you have disconnected your line. The remaining amount will be
refunded to you 90 days after the last billing date.
You will receive a monthly statement to advise you of your charges.
Campus Mail your Cheque or Money Order, along with the slip from your monthly
statement, to our office. Campus Mail is free and easily located in all residences.
Walk in to our office and pay with your Debit Card, Cash, Cheque, or Money Order.
UBC ResTel Service Contract
1. INTRODUCTION
This residence telephone agreement constitutes an offer by the University of
British Columbia (herein after UBC or the University), through Information Technology (herein after IT), to contract with the Student in residence (herein after referred
to as the Customer) to provide telephone service in a student residence on the
UBC Campus.
The customer may accept this offer by completing and signing the ResTel Service
Application form and returning it to ITSC along with a deposit made payable to
“The University of British Columbia.”
2. ELIGIBILITY
At this time ResTel is only available to the residents of Thunderbird, Ritsumeikan,
St. Andrew’s Hall, St. John’s College and Vancouver School of Theology
Residences.
3. CONTRACT TERM
This contract will remain in effect until the Customer advises IT in writing of their
desire to terminate service. Five working days advance notice is required.
4. UNIVERSITY RESPONSIBILITIES
The University agrees to provide telephone service and certain telephone
products to resident students for the length of the contract.
5. CUSTOMER RESPONSIBILITIES
The Customer agrees to make payment for all telephone services that they choose
to use and any telephone related goods that they choose to rent or purchase from
IT.
The Customer is responsible for ensuring that their private telephone equipment
is in good working order and will not contribute to any degradation of their
telephone service.
IT will charge a service fee to cover material and labour used to investigate any
service trouble call that proves to have been the fault of customer-owned
telephone equipment. Please check your equipment before registering a service
trouble call.
6. PRICING
IT reserves the right to change the price of any goods or services provided to its
Customers. Notification will be made 30 days in advance of any pricing changes.
7. LIABILITIES
IT does not guarantee uninterrupted working of the ResTel service or any telecommunication equipment purchased from IT in connection with this service. As a
result, the University is not liable to the Customer or any others either directly or
indirectly as a result of such failure.
The University is not at any point responsible for lost, stolen, or damaged property
belonging to the Customer including property located on campus and goods in
storage. Nor shall it be responsible for any injury, damage or loss whatsoever
caused to the Customer, his or her guests or invites while in or about the residence
of the University campus.
The Customer shall not do or permit to be done any act or things which may
render void or voidable any insurance policy of the University.
The Customer shall indemnify and save harmless IT and the University, its Board or
Governors, directors, officers and employees, from and against any expense, loss
or damaged suffered by IT or the University by reason of negligence, or any breach
of non-performance by the Customer of any term in this contract.
8. REFUND & FORFEITURE POLICY
Installation, extension service setup and directory fees are non-refundable. ResTel
and Voice Mail service fees are refundable if written notice of cancellation is
received by IT before service commences.

The forward payment of ResTel and Voice Mail service fee is refundable and is
calculated from the date of departure. Written notice of cancellation is required
5 working days in advance by IT.
9. DEPOSIT REFUND
Deposit refund can be arranged by cheque. The following conditions will apply:

Only refund amounts over $10 can be accomodated
Cheques mailed to a Canadian or US address will be issued in the
country’s respoective currency.
For international addresses, a US bank draft will be issued to the
customer if the final balance is over $10 after a $7 wire transfer fee is
chared.
10. DAMAGE, REPAIR & SERVICES CHARGES
Tampering with communication cable, cable jack outlets, telephone and data
equipment of communication rooms is prohibited. The Customer agrees to pay
for any damages, lost property and service or administration costs caused by the
Customer to University residence communication facilities through accident,
neglect or intent.
11. TERMINATION & CANCELLATION SERVICE
The Customer may cancel the contract at any time by giving five (5) working days
notice in writing or in person to IT.
The University may terminate or temporarily suspend this contract without notice
in the event of a situation which makes continued operation of residence
telephone service unfeasible or for any other reason it deems desirable at its
discretion.
The University may cancel or terminate this contract if the Customer fails to meet
the full terms and conditions stated herein, or if the Customer violates University
and/or residence regulations and standards.
The University may cancel or terminate this contract to preserve the integrity of
the network, comply with laws or if the Customer abuses or misuses the
telephone service.
12. SECURITY
IT has introduced authorization codes to place long distance telephone calls.
The Customer is responsible for the security of their authorization code and is
responsible for all changes made in connection with its use.
13. INTEREST ON SECURITY DEPOSITS
IT will pay interest at a predetermined rate on security deposits.
14. LATE PAYMENT CHARGES
Failure to settle your telephone account on the due date will result in a 1.5% per
month (18% per year) late payment charge being added to your next monthly
statement. Customers with overdue accounts may be disconnected.
15. RECONNECTION FEES
Reinstatement of telephone service will only be possible when all arrears plus any
initiation fees and a $30 reconnection fee are paid in full. IT does not guarantee
the availability of requested telephone numbers.
16. TRANSFER OF AGREEMENT
The Customer will not transfer this agreement and the service provided to them
to anyone else unless the University agrees in advance in writing.
17. EXTENT OF AGREEMENT
This is the entire agreement between the University and the Customer. UBC does
not make any other representation or guarantees regarding the goods and
services provided to the Customer.
