I hereby instruct you to prepare my Financial Statements and related tax returns until further
notice. I do not require you to complete an audit or review of my Financial Statements and
understand that the standard disclaimer recommended by the New Zealand Institute of
Chartered Accountants will be attached to the Statements.
I certify the information provided is a true and complete summary of information you require to
complete this assignment.
I acknowledge that the responsibility for the accuracy and completeness of the information
remains my responsibility.
I accept responsibility for all records and information supplied to you and any failure to supply
you with all records and information.
I authorise you to contact my Bank, Solicitor, the Inland Revenue Department and any other
party in order to obtain any further information that you may require to complete my return of
income and financial statements. I acknowledge that this information would not otherwise be
available due to the Privacy Act restrictions, but I give my full authority for this statement to be
used as written confirmation of my agreement to your obtaining information from the Bank,
Solicitor, the Inland Revenue Department and any other party for the above mentioned
purpose.
Accident Compensation Corporation
I authorise your organisation to act as your agent for ACC levy purposes for all associated
entities. This authorisation allows your organisation to query and change information on our
ACC levy account(s) through ACC staff, and through ACC Online Services. This authority will
also allow your organisations’ main representative discretion to delegate access to our ACC
information to other members of our organisation. Other delegated members of your
organisation will also be able to query and change information on your ACC levy account.
I acknowledge and agree that, as part of the on-going quality control initiatives of the New
Zealand Institute of Chartered Accountants, files relating to assignments completed on my
behalf may be examined on a random test basis by an official reviewer by the Institute for that
purpose.

With reference to the Privacy Act 1993 we advise that the information which has been collected
for the purpose of completing your Inland Revenue Return of Income and the information
collected for this purpose will not be disclosed to any other person except in the event of a

I/we give authority to Colbert Cooper Limited (our tax agent) to act on my/our behalf for
all tax types until further notice.
Authority is given to obtain information from Inland Revenue and other financial
institutions to enable my tax returns to be completed. This includes obtaining
information via online services available on Inland Revenue’s website.

All costs are inclusive of GST unless otherwise stated.
2.
Payment of services shall be paid to Colbert Cooper Limited by the 20th of the
month following the month in which the invoice is dated, unless otherwise agreed
by both parties.
3.
Interest at 1.5% per month may be charged on overdue accounts.
4.
Any expenses, costs and disbursements incurred by Colbert Cooper Limited in
recovering any outstanding monies including debt collection agency fees or
solicitor costs shall be paid by the Client.
5.
The Client authorises Colbert Cooper Limited to collect, retain and use personal
information about the Client (including the information collected in this document)
for the following purposes only:
(a) Assessing the Client’s creditworthiness.
(b) Disclosing to a third party details of this application and any subsequent
dealings it may have with Colbert Cooper Limited for the purpose of
recovering amounts payable by the Client and providing credit references.
(c) Providing the professional services as agreed with the Client, including
contact with Banks, Inland Revenue Department, Solicitors and other parties
as required.
