HEALTH FAIR PROJECT REQUIREMENTS
This project will be discussed in greater detail in class during the first cycle of the semester
PROJECT OPTION 1: PHOTO JOURNAL
THEME “How I Build My Total Health Triangle”
GOAL: Students will demonstrate how they are actively contributing to the positive
health of their community as a whole. Daily chores, jobs for pay or school credit do not
apply.
DUE DATE – Your project due date is the first Friday of the last full month of class.
HOW WE WILL USE YOUR PROJECT – Your project will be entered into the pool of projects
The Peer Mediation Team will have to choose from for display in The Health Fair held at
CHS each May.
GENERAL CONTENT REQUIREMENTS FOR ALL PROJECTS
A Must have a minimum of 12 photos that reflect YOU performing 12 DIFFERENT
activities that support the various positive life concepts taught in our health/fitness
curriculum.
o The 12 photos should be equally divided to represent the 3 sides of your
health triangle – that is…
ƒ Physical Health – 4 photos
ƒ Mental/Emotional Health – 4 photos
ƒ Social Health – 4 photos
B Photos must have been taken within the past year.
C Each photo must have a caption of at least three sentences that explains the
relationship between the project theme and the specified photo.
D Each photo should represent a separate activity or category of activity in the
student’s life. For example, if your family does a lot of conservation, recycling
and/or energy reduction, that counts as ONE activity and therefore, ONE photo.
You MAY use more than one photo per activity on a single page if you choose to
do so, but it will only count as ONE topic of the TWELVE required.
E Your project should send a positive message of supporting healthy living.
F Be sure to give visible credit to any photographers/hardware/software programs
you used to create your project, otherwise it is plagiarism (see CHS Code of
Conduct). This may be done on each page or in a single page bibliography at
the end of your project.
OPTIONS AND REQUIREMENTS FOR CREATING YOUR PROJECT
A. PHOTO JOURNAL REQUIREMENTS – ALL PROJECTS
1. Each photo or group of themed photos should be mounted on a
separate page (one-sided).
2. Be sure to put your name on the outside or inside cover with your period
number!
B. OPTION 1: HARD COPY PROJECT
1. Acceptable Presentation Folders: Hardback Scrapbook, Photo album,
Journal or Slim 3-ring binder with transparent page covers.
2. NEATNESS: Avoid smears, stains, uneven paper cuts, uneven spacing, etc.
Captions should be printed in neat block letters or typed.
3. MOUNTING/DISPLAY: All materials are to be neatly and securely fastened.
Use of double-sided tape is recommended as visible tape/glue takes
away from the overall neatness of your project. In our experience, puffy
paints and glitter take away from the neatness of your project as they
tend to smear, stick together and tear photos.
4. DURABILITY: Remember – your project may be accepted to the Health
Fair. If so, it will be handled by the high volume of visitors we have to the
event each year. Be sure your project can hold up!

6. WE HIGHLY RECOMMEND THE USE OF PDF Formatted Files if your project is
very large! In addition, it will secure your content.
7. ADDITIONAL ELECTRONIC PROJECT REQUIREMENTS
a. It is your responsibility to be sure your project is formatted so it can
be viewed in your Health Classroom by your instructor.
b. If you are using a web-based format, you are required to provide
access information to your teacher on or before the project due
date via the project evaluation form. Remember to include any
special instructions about accessing the project such as log-ins,
passwords, etc.

Check again to be sure you’ve put your name, teacher name and class period
number on the first or last page of the project (first page/slide is preferred).
Be sure to put your name at the top of the PROJECT EVALUATION FORM (in this
notebook) and TURN IT IN to your classroom teacher WITH YOUR PROJECT on or
before the due date.
TURN IN OPTIONS:
o In the classroom area designated by your teacher (hard copy or
electronic projects saved to flashdrive or disk)
o Synergy upload – see details below
o Project Evaluation Form – if web-based, put address on form and turn into
designated area
If you’ve created a blog or some other form of WEB-BASED electronic project, be
sure to record the information needed for the teacher to view your project on the
PROJECT EVALUATION FORM in the space provided.
All projects and evaluation forms are to be placed in the appropriate
Health/Fitness Synergy project box or designated space in your classroom on or
before 2:30pm on the due date to receive on time credit. Remember – if you turn
in your project through SYNERGY you have until 11:59pm on the date due to turn
it in for on time credit. After that time, you will be locked out of Synergy and
required to hand in a hard copy of your project. Hard copies of electronic
projects should be handed in on a CD, DVD or Flash/Thumb Drive to your
classroom teacher. Be sure to pick up the hardware you give us. Most of the time,
your classroom teacher will want to download your project to Synergy and hand
the hardware back to you immediately if time permits.
IMPORTANT: Bonus Points will be awarded for projects turned in up to 5 days early.
You will receive up to 1 point per day early prior to grading of the project.

DUE DATE – Your project due date is the first Friday of the last full month of class.
PROJECT THEME: Choose any topic listed in the course content provided earlier in this notebook,
narrow it down to a specific storyline, then seek your teacher initials for approval below.

HOW WE WILL USE YOUR PROJECT – Your project will be entered into the pool of projects The Peer
Mediation Team will have to choose from for use in the STAR Program. This is an educational
outreach program in which the mediators teach the elementary and middle school students in the
TE School District.
GENERAL CONTENT REQUIREMENTS FOR ALL PROJECTS
A Must have a minimum of 11 pages in total
1. Page 1 should be a colorful book cover that includes a creative and original title,
the author’s name and school.
2. Page 10 is your Bibliography page.
3. Page 11 is the “back cover” or last page and should contain a short “bio” about
the author – suggestions that elementary/middle level students like of you, the
author, your grade, interests and why you chose the topic. The last page may also
include a summary of the story and - just for fun – “fake” quotes from the news
media about how wonderful the book is, maybe a “fake” ISBN number or a
barcode to make the book appear more “real”.
4. Pages “2-9” are the required pages of story content. If you are unable to wrap up
your story in 8 pages, you may choose to add length, but keep the length and
reading level age-appropriate. Your teacher will explain more about this in class.
B Remember that your story should include accurate information with a positive message –
scare tactics are inappropriate and are rarely effective in preventing undesirable
behaviors. If you think about why you chose the topic, it may help you to write your story.
C NEATNESS: Remember that our youngest students are accustomed to reading print fonts.
The size of the font decreases with age so it is crucial to consider your audience age.
D LAYOUT: The younger the student, the more pictures will help to tell the story.
E DURABILITY: Our youngest students are also very hard on your books. While the mediators
are usually the ones to handle hard copies, there are times when classroom teachers ask
to borrow these books. If that is the case, these tiny hands WILL handle your book!
Therefore, we STRONGLY recommend electronic copies of your book. If you provide a
hard copy, we recommend it be bound in a hard cover notebook with transparent covers
for each page.

Your project should send a positive message of supporting healthy living and the topic you
have chosen should be discussed in an “age-appropriate” manner.
G Be sure to give visible credit to any artists, electronic art, photography, or
hardware/software program you have used to create your project. Failure to do so is
plagiarism (see CHS Code of Conduct). Sources in the Children’s Book should be provided
on an additional page PRIOR to the (last page) back cover of your book.

LAYOUT/DESIGN: We strongly suggest you look at various books in your public or
home library to determine the font size, length of sentences, length of individual
words for your target audience. These books will also help you to choose various
layouts. Remember – pictures can change the meaning of your sentence –
choose wisely!
4. DURABILITY: Remember – your project may be accepted for use in the TESD
classrooms. If so, it will be handled by a high volume of rough handlers! Be sure
your project can hold up!
