health care reform for our state and nation.
Partnering with our patients, community,
allies and staff is vital to the success and
expansion of the service, capacity building,
and the advocacy results we produce.
From the
President
Forty-five years ago the Cornell Scott-Hill Health
Center, or Hill Health Center as it was first known,
opened its doors on Columbus Avenue and became
a part of the Hill community here in New Haven.
From day one, you welcomed us with open arms,
inviting us into your lives and your families. You
made us feel like we were part of something bigger.
And for that we are very grateful.
Since then we have grown and expanded, and now
serve communities throughout New Haven County.
In total, we operate nineteen care sites including
school-based health centers and imbedded primary
care clinics at community partner sites. And at each
one, just as with the Columbus Avenue site, we have
been welcomed with open arms.
We are welcomed because we bring valuable
services to the communities that we join. We provide
them with medical, dental and mental healthcare
as well as host of specialty services. Additionally,
we offer groups and classes covering everything
from smoking cessation to art therapy. These are
all important resources that the people of these
communities may not otherwise have access to if
we didn’t exist. In fact, patients often tell me just
that. They say, “Where would I go if the health center
wasn’t here?”
To those who ask, I say, “Do not worry. We are not
planning to go anywhere.” And we’re not. We are
committed to serving the communities that we
already do and hope to continue our path of growth
so that we can serve even more people in need.
However, we can’t do it alone. Your support is
needed. That is why I am asking you to please
consider making a donation to the health center.
Your money will go toward valuable programs such
as our pharmacy fund, which provides financial
assistance for individuals who are experiencing
difficulty in paying for medications.

Thank you…that is often the message we receive
from you, our patients. In fact, throughout the year,
we receive many notes of gratitude from you and
other members of our community. Some are thanking
us for the services we provide. Some are thanking us
for giving them a hand to hold. Some are thanking
us for helping them navigate the difficult world of
health insurance. And some are just thanking us for
providing a smile when they needed it.
But now it is our turn. It is our turn to say thank you
to you.
Thank you for inviting us into your community. We
are so very proud to be a part of so many wonderful
communities here in New Haven as well as in West
Haven, Ansonia and Derby; communities that are
rich in history and filled with promise for the future.
It is truly an honor to be part of something bigger.
Thank you for inviting us into your family. One of my
great joys is seeing the generations of families who
walk through our doors. Men and women who came
here for care when we first opened are now bringing

their children, and in some cases their grandchildren,
here for care. It is very rewarding to know that we
have touched the lives of so many generations.
Thank you for inviting us into your life. Healthcare is
a very personal thing. There are moments of great
personal joy and times of extraordinary worry.
Thank you for letting us be there to celebrate with
you or to hold your hand when you need it. It is a
privilege to be there for you no matter what your
circumstances are.

“Hi, I’m Jillian Skinner,
a Senior Dental Assistant/Supervisor
here at the Cornell Scott-Hill Health Center’s
Columbus Avenue Dental Department. For the past twenty-six years I have been
a part of this community, a part of this health center and a part of your family.
Each day, I have the privilege of working with you, your family, your friends and
all those who seek care with us.
As a dental assistant, I am on the front lines, working closely with our dentists
and hygienists to help you change your life by changing your smile. There
are the things you see me do: taking x-rays of your teeth, assisting the
dentists during procedures and sterilizing the instruments. There are the
things you don’t see me do: supervising the other assistants, ensuring the
proper maintenance and function of the dental equipment and machines,
maintaining dental logs and material safety data sheets, ordering supplies and participating in
infection control meetings, to name a few. Then there are the things that matter most.
My job is about so much more than taking care of your teeth or maintaining our equipment. My job is to hold your
hand when you are nervous. My job is to explain things to you when you don’t understand. And my job is to help you
see the beauty within yourself.
And today, thanks to the health center’s extraordinary growth, I am able to help not only you but so many others. The
health center’s dental department has undergone significant expansion in recent years. Now, we are able to see more
people and offer more services. That means we are able to have an even greater impact on this community and for
that, I am very thankful”
“Hi, I’m Zulma Garcia,a Customer Service Representative for the Cornell Scott-
Hill Health Center’s Call Center. I joined the health center about eighteen months
ago after relocating to New Haven from New York and I couldn’t be happier.
That’s because each day I have the opportunity to make a difference in your life,
in the lives of everyone we serve.
As a Customer Service Representative, my primary responsibility is to answer the
phone when you call. But that is not my job is its entirety; it is only the beginning
of what I do. Whether you are calling the health center to make an appointment,
to get a prescription refilled or to connect with another staff member, my job is
to connect with you.
That means listening to what you are saying. That means treating you with
respect and courtesy. That means helping you in any way that I can. That means
exceeding your expectations. That means working with you to resolve any
issues. And that means following up with you to make sure that your needs
were met.
I come to work each day and give 100% because that’s what you deserve, that’s what our community deserves and
that’s what our family deserves. And I know it is appreciated. You show your gratitude not only through words of
thanks, but by calling and asking to speak with me directly. That’s how I know I have connected with you. That’s how I
know I am making a difference in your life.”
“Hi, I’m Frujen Bridgewater,a Patient Account Representative here at the
Cornell Scott-Hill Health Center. I joined the health center last September,
making me one of the newest members of the team, the community and the
family. However, I already feel right at home.
In my role as a Patient Account Representative, one of my main responsibilities
is to make sure that payments are allocated to the proper patient accounts. I
also interact directly with patients such as yourself on issues related to co-pays
and self-pays. This is an important and rewarding part of what I do.
Whether it is answering your questions or helping you understand payment
options, I know that I am making a difference; making what can be a very
stressful subject easier to understand. I value that opportunity and am
thankful to be an integral part of the process.
I am also thankful to be a part of this community as well as the larger
community of nonprofits. I have worked for nonprofits since 2004 and believe that they play a vital role
in society. The health center, for example, provides much needed medical, dental and mental health services to those
who may not otherwise have access to or the ability to pay for such care.
Getting to be a part of that is something I value greatly. Just as I value you and all those we serve.”
“Hi, I’m Heidi Biegel,a Nurse Midwife in the Cornell Scott-Hill Health Center’s
Columbus Avenue OB/Gyn Department. I joined the health center in 2009
and quickly realized that I wasn’t just joining the health center. I was joining a
community, a family.
As a Nurse Midwife, I work directly with women like you who seek care in our
department. I provide not only the services you need such as annual exams
and prenatal care but I am also here to offer a listening ear and a hand to hold.
I understand that your life is complicated and multi-faceted, and your body is a
reflection of that. I am here to help you with all of your needs.
I want you to know that I am so appreciative of you and all the women who
come to us for care, tell us their stories, and allow us to work with them to foster
wellness in their lives, their families and their communities. I am inspired by you
and others who face significant health challenges with courage, strength and
often creativity and humor.
And while I am there to help during times of challenge, I am also honored to be
there for you during times of happiness. Working with you and your family during
your pregnancy is such a joy. It is an honor to be invited into your life at such an
important time. One of my greatest sources of pride is our family photo gallery. Located on the
wall outside our waiting room, it showcases women like you who successfully
attended prenatal care and their postpartum visits and were invited to have
a family portrait taken. Just walking by those photos, you can see the vast
configurations of love and family that we see in our clinic. I want to thank
you and your family for all that you do to contribute to a healthy family and a
healthy community. And thank you for letting me be a part of it.”
“Hi, I’m Elizabeth Carrena,Director of Nursing for the Cornell Scott-Hill
Health Center. I joined the health center eighteen months ago and couldn’t
be happier. I finally feel like I’m home.
I grew up not far from here in Bridgeport, Connecticut in a neighborhood similar to the Hill neighborhood where our
main care site is located. Growing up, I always knew I wanted to be a nurse so I worked hard to gain the education and
skills I needed to achieve that dream. Since becoming a nurse, I have worked in a variety of settings, all with one goal
in mind: to become the Director of Nursing for a community health center. I wanted and needed to return to my roots.
And now I have.
As the Director Nursing, I guide clinical practices to ensure that you, our patients, are receiving the highest quality of
care. Additional areas of focus for me include regulatory compliance, customer service, mentoring and technology. But
this is not about the work I do, it’s about the feeling I get when I am here. I feel like I am a part of your family.
The health center is a unique and amazing place. It is a safe haven for you and all those who seek care here. It is a place
where true partnerships are formed. It is a place where a real difference is being made.
And that is thanks to you. You have helped make us who we are today. You have done this by expressing your needs
so that we can best meet them. You have done this by taking an active role in your healthcare and working with us to
keep you well. You have done this by welcoming us into this community, into your family. You have truly been the key
to our success. So thank you. Thank you for all that you do to make us who we are.”
“Hi, I’m Jason Black,and I serve as the Program Director for the health center’s
Columbus Avenue, West Haven and Ansonia behavioral health care sites. Seven
years ago I joined the health center as a Clinician. Since then, my responsibilities
have grown and my position has evolved. However, what’s most important
hasn’t changed: Each day I still get to come to work and make a difference in
the lives of the people we serve.
As a behavioral health provider, I work directly with people in the community;
people like you who need someone to talk to, someone to listen and someone
to care; people who need a place where they don’t feel judged, a place where
they can heal, a place where they can get the tools and skills they need to
move forward on their journey successfully.
Unfortunately, people who seek behavioral healthcare often face additional
challenges such as a lack of resources or cultural stigmas about treatment.
Sometimes they are struggling to just survive one more day. It is an honor to
work with a team that helps those in need overcome these barriers. I am proud of the quality
of care that we provide you and other members of this community. And I feel immensely privileged to work for an
organization that not only provides these needed services but does so in a culturally-sensitive and appropriate way.
However, the greatest rewards in this work come from our clients themselves. I have learned as much from them as
they have from me -- their strength and resilience is a constant source of inspiration.
I am inspired to continue helping people by helping ensure that we provide excellent healthcare. To that end, I am
excited to be a part of our work to further integrate the health center’s behavioral
healthcare with its other services – primary care, wellness and outreach, dental, the
list goes on – so that the whole person is treated. After all - we aren’t here to treat
diabetes or heart disease, substance abuse or depression – we are here to serve
and to treat people, the people of this community, our family. When we all work
together we have the greatest impact and achieve the best outcomes.”
“Hi, I’m Sharon Tyson,
Administrative Assistant for the Cornell Scott-Hill Health
Center’s Infectious Diseases Division. I have been with the health center for four
years now but am not new to this community, this family. I was raised here in
New Haven and have stayed connected to the City throughout my career. In
fact, for the past thirty-one years I have worked in and served many of the City’s
neighborhoods including the Hill neighborhood.
In my role as an Administrative Assistant, I am often your first point of contact. I answer the phone when you call. I take your messages. I assist in helping you schedule your appointments. And I
provide you with care coordinating services. But this is more than a job; it’s about being part of a community, part of a family. That is why when you call, I listen.
That is why I offer compassion when you need it. And that is why I don’t judge. I am here to help you; to connect you to the services you need and to ensure that you get the care you deserve, in the
community where you live.
The health center is a tremendous resource for this community. It offers countless programs and services all designed
to best meet your needs whatever they may be—medical, dental or behavioral health. But it is so much more than
that. It is an integral part of this community, this family, providing people with a place to hope, a place to heal and a
place to call home.”
“Hi, I’m Rose Swift,Development Director, Grant Programs & Research for the
Cornell Scott-Hill Health Center. For the past twenty years I have worked for the
health center, helping to bring you the programs and services you need.
While my role is largely behind the scenes, I want you to know that I am working
hard every day to make a difference in your life. I work closely with people
throughout the health center to identify needs in this community, this family,
that aren’t being met. Then, I put on my research hat and scour all available
resources to find funding to support programs and services that can meet
those needs.
I come to work every day because I am committed to you, your family, your
friends and everyone who walks through our doors. I am committed to
bringing you the programs and services that you might not otherwise have
access to.
When a grant gets funded, I feel like it is Christmas morning because I know that you
will now be able to get services that you weren’t able to get before. For example, I can readily recall the joy I felt when
we received the public housing primary care grant. This grant allowed us to bring primary care services to you and
others that live in public housing, a population we hadn’t been able to reach before. And when we received the Birth
to Three grant, well, it is difficult to explain the elation I felt. Thanks to that grant, you and parents just like you, are able
to get much needed services for your children right in the community where you live.
I don’t work directly with you, our patients, but I know that I am part of your family. I am making a difference in your
life and that’s what families do.”
“Hi, I’m Amos Lopez,a Customer Service Representative for the Cornell Scott-Hill
Health Center’s Call Center. I joined the health center three months ago, but have
been a part of this community, this family, my entire life.
I was born and raised here in New Haven and have spent most of my life working
in the community, giving back in as many ways as I can. For example, I am very
involved in my church and do a lot of ministry work throughout New Haven.
I believe that the work I do for the health center is an extension of the work I am
already doing in the community. It is another way in which I can make a difference.
When you call the health center, I am able to help you. I can put you in touch
with the right person. I can connect you to a valuable resource. I can answer your
questions. Even something as simple as scheduling an appointment for you is
rewarding because I know that I am playing a part in positively impacting your
health, your life and your family.

The Pharmacy Fund – provides financial assistance
for individuals who are experiencing difficulty in
paying for medications.
Cholesterol Testing Machine – $2,500
Help us conduct free screenings at health fairs and
at other outreach events. This machine measures a
complete lipid profile plus glucose in only 5 minutes
using blood from a simple finger-stick.
The Patient Care Fund – provides financial
assistance for individuals who are experiencing
difficulty in paying for medical, dental or behavioral
healthcare services.
The Insulin Bank – provides financial assistance for
individuals who are experiencing difficulty in paying
for diabetic supplies and medications.
Transportation Services – provides transportation
to individuals who are experiencing difficulty in
getting to or from their appointments.
Village of Power/Growing Through Sewing
– provides services to women struggling with
substance abuse, homelessness, mental health
challenges, or HIV/AIDS. Growing through Sewing is
one of the recovery programs offered at the Village
of Power. Through the program, women learn skills
such as sewing and jewelry making that build their
confidence and aid in the recovery process.
Passenger Van – $23,000 (*Branding opportunity)
Improve our ability to transport the people we serve.
We all know transportation is a barrier to care and
this van will greatly benefit the people who walk
through our doors.
Waiting Room Information Center – $2,500
(*Multiple branding opportunities)
As people arrive for their appointments, help us
reach them with important messaging on healthrelated topics. We also can share critical reminders
on how to book an appointment, hours of service,
treatment options and other information designed
to improve their overall experience.
Treatment Room Literature Racks – $15,000
(*Multiple branding opportunities)
Help us share important health information with
patients and clients. These literature racks are
installed in all of our treatment rooms making it
easier for our providers to hand brochures directly to
the people they serve.
