APPLICATION FOR MEMBERSHIP
Presentation of the attached material is an important part of the application process for the
National Honor Society. It is expected that entries are typewritten and presented in a neat,
orderly and well thought-out manner. It is strongly suggested, that each candidate use
technological resources to produce their application portfolio ensuring that all information
requested is provided. Check each section of the portfolio before submitting::

4. _____ Student Activity Form, a record of student accomplishments with appropriate
signatures. Included in this section must be co-curricular activities, community
activities, work experience, leadership positions, and service activities.
5. _____ Academic History (you can get this from your counselor).
Materials must be put in a large manila envelope, with your name at upper right corner, and
submitted to an NHS Sponsor by the deadline: September 10, 2014 by end of school.
I have been informed that I am academically eligible for membership in the National Honor
Society. I offer the following information as fulfillment of the requirements necessary for
consideration.
If selected, I agree to abide by the constitution of the National Honor Society and the bylaws of the Englewood High School Chapter, to pay the $20.00 annual membership fee, and
to become involved in at least two (2) projects planned by the NHS leadership.

CANDIDATE’S ESSAY
Discuss the qualities that you possess that you feel make you eligible to be a member of the
National Honor Society:
The four pillars of National Honor Society are:
Character, scholarship, leadership, and service.
Write a personal essay, between 350-500 words, double-spaced, that
explains why you would be an effective NHS officer or general member. Remember
that we are seeking students who demonstrate excellence in scholarship, service,
character, and leadership. Use this essay as an opportunity to tell the NHS Faculty
Council more about yourself and why you should be chosen for the program. Ideas
may include:
My accomplishments,
My failures (and how I corrected them),
A specific time I exemplified leadership (and lessons you learned),
My understanding of the NHS motto Noblesse Oblige (the moral obligation
of those of high rank to display honorable and generous conduct) and/or,
A detailed example of how I would carry out a specific service project.
Please use this format for your typed essay: 12 point Times New Roman, doublespaced, with 1” margins.
Be sure to have your name at the upper-right hand corner of each sheet.
