Payment Of Account
– Accounts are due in full by the 25th of the month following date of statement.
We add a 1.5% late charge on all past due balances.

Billing Errors

Lab Errors
– Billing errors should be reported within 30 days of the invoice date.
– a) Remakes for lab errors must be ordered within 30 days from date of origin.
b) A specific reason for remake must be stated.
c) Credit will be issued upon receipt of the original frame and or lenses and original invoice.
Doctor’s Errors
– a) Remakes for doctor’s errors must be ordered within
90 days from date of origin.
b) Remakes for finished Rx’s will be billed at 50% as long as there are no additions.
The credit will only be for the original amount...
c) Uncut remakes will be billed at 50% for prescription
changes only upon return of original lenses.
d) Progressive nonadapt – there
will be a 90-day period in which we will issue full credit or remake the lenses
at no charge upon reorder or return of replacement RX. Progressive lenses that have Teflon, Avance, Alize’, Crizal or Digitally processed will be redone at 50% .

Returned Merchandise – the original invoice must accompany Lenses being returned for credit. We will not credit merchandise over 30 days.

– We cannot be responsible for the condition of enclosed lenses. When replacing one lens only (1/2 pair) inspect the other lens carefully for scratches and chips.
– Reasonable balance lenses will be supplied at half the regular base price. Exact balance lenses will be supplied at full price.
Special Lenses – Add-on prices and extra charges are not applicable to the pricing of unusual and special order lenses. Please call for price quote.
Re-edge/Re-surface Glass Lenses – We cannot re-edge or re-surface glass lenses due to strict F.D.A. impact resistant specifications.

Frames – We are not responsible for old frames, which may break in the process of insertion. We are also not responsible for frame alterations.
Direct Sell Frame Policy – There will be an $12.00 service charge on stock frame orders from direct sell manufacturers.
Lost or Damaged Merchandise – We cannot be responsible for merchandise shipped to us that we do not receive or that is received damaged.
Handling Charge – There will be a $2.00 per invoice handling charge to help offset the ever-increasing costs of shipping, handling, and delivery. Postage paid labels and envelopes are available upon request

Prices – All prices are subject to change without notice. These prices supercede all prices previously given.
RX Warranty Program
Each customer must specify on each Rx that he/she desires the warranty program, or a customer’s account can be set up to include the warranty program on each Rx.
All finished Rx’s are eligible, including lenses only. Uncut lenses are not included.
The cost is $1.50 per Rx warranty, billed at the time the original invoice is issued.
Coverage – For a period of one year from the date of the original invoice, Allentown Optical Corp. will:
Replace any broken lens or lenses identical to those originally ordered at no charge. We must
receive the broken part or parts with a copy of the original invoice or the original invoice number
and the date in order to begin replacement.
Replace or repair any part of a frame that is broken or damaged, provided it was supplied by
Allentown Optical Corp. or the whole frame replaced by another frame or part of a frame at no
Charge. We reserve the right to substitute an equal or better frame should the original frame be
Discontinued by the manufacturer
This Rx warranty program does not cover theft, loss of glasses, or scratched lenses
Patient Satisfaction Warranty $5.00-Coverage-We will replace the RX for a period of one year, one time for any reason up to the original invoice
price, must be asked for at the time Rx order is placed. Any extras will be billed in full at time of replacement. N/A on Digital Varilux Products.
There will be no warranties on glass lenses, safety lenses or metal grooves.
