Green Information Technology (Green IT) Survey
Thank you for taking the time to complete this Green IT survey. Your views on how to reduce
organizations’ Information Technology (IT) carbon footprint (Carbon dioxide or CO2 emissions
and/or carbon neutral, green house gases (GHG), climate change) are vital to the development of
better solutions in the IT life cycle and particularly their effect on our environment locally,
nationally, and globally.
Instructions for completing the survey:
For questions that require a written response, write your response in the space that is provided, for
all other questions, you may circle the appropriate answer. After you have completed your
questionnaire, please submit your responses electronically to Karen.Bolch@gov.bc.ca. You may
also attach any documents or links that you feel would be beneficial to this study.
Privacy:
To respect your privacy, this survey is confidential and answers will only be used on an
aggregated basis. I will only be sharing your answers with a select few people within the Office
of the Chief Information Officer who require the information as part of their employment duties
(i.e. Sponsors and advisor).
Should you choose to participate, any personal information collected from you is collected
pursuant to section 26(c) of the Freedom of Information and Protection of Privacy Act, as it
relates directly to and is necessary for the completion of my thesis and the B.C. government’s
goal of reducing organizations’ IT carbon footprint. When providing information to us, we ask
that you do not provide the personal information of other people.
Personal Information: Recorded information about an identifiable individual, including but not
limited to the individual's name, address, telephone number; race, national origin, colour,
religious or political beliefs; age, sex, sexual orientation, marital status, family status;
fingerprints, blood type, disability; educational, financial, criminal, or employment history;
anyone else's opinion about the individual, or the individual's personal opinions, unless they are
about someone else; an identifying number, symbol, or other particular assigned to an individual.
You have the option to back out of the survey at anytime by sending an email to
Karen.Bolch@gov.bc.ca and stating you want to withdraw from the survey in the subject line.
Your results will not be used if you choose this option. If at any time you have questions or
concerns about our collection of personal information or any other matter, you may contact me at
