National Honor Society Self-Nomination Fall, 2008 National Honor Society exists to recognize students who demonstrate excellence in the four “pillars” of scholarship, leadership, service, and character. The self-nomination process provides an opportunity for you to demonstrate your high achievement in each of those areas. To nominate yourself for induction into Trinity High School’s chapter of the National Honor Society, provide the following information in a neat and thorough manner. Use your word processing program to create the space you need for each response. Type your answers in complete sentences and paragraphs, using correct language reflective of your commitment to excellence. Then obtain the requisite signatures, and submit the materials to the Attendance Office. If you have any questions about National Honor Society or about this self-nomination form, see Ms. Kovacs. Deadline: November 24, 2008, 3:00 p.m. Name: _________________________________ Grade: 11 12 Scholarship: minimum 3.4 GPA Note that this is a minimum. If your GPA is under 3.8, be especially careful to explain how your academics do reflect actual excellence. What was your over-all GPA in June, 2008? _____ What was your average for the half-term that ended in October, 2008? _____ How have you demonstrated academic excellence in your course selections and in your performance in and out of the classroom? Leadership: the ability and willingness to take charge This may or may not involve formal leadership positions. Leaders are the people who step forward to make classroom projects, class activities, ministries, sports competitions, and extracurricular events successful. How do you manifest leadership here at Trinity? How do you manifest leadership outside of Trinity? Service: giving time, energy, and resources to help others Service hours required for graduation are certainly part of this, but they are not the whole picture. Excellence in service means giving of yourself even when you personally have nothing to gain from the situation. Sometimes service benefits individuals in need; other times it ministers to entire groups. How do you extend yourself in service within the Trinity community? How do you extend yourself in service outside of the Trinity community? Character: integrity, fidelity, love, justice, maturity Character involves what one does and what one avoids; it also involves what one is. Of course, no one is perfect. Excellence in character generally means that people recognize in you characteristics such as trustworthiness, strength, generosity, integrity, honesty, love, justice, determination, and maturity. Describe three components of the excellence of your character. References Obtain the signatures of three Trinity teachers or administrators who can attest to your excellence in scholarship, leadership, service, and character. _____________________ _____________________ _____________________ Student’s and parent’s/guardian’s signatures I understand that admission to National Honor Society is based on approval by Trinity teachers and administrators.
