Registered Body copy, the address
will be the Registered Body’s.

Applicant. Please note the name
given in B2/3 on the application
form is the one that will appear on
the certificate.

Disclosure
Scheme Membership Statements
will only have an applicant copy
unless the application was
countersigned.
Disclosure Number, which is unique to
each Certificate (Applicant and
Registered Body copies will have the
same numbers). Date of when the
Disclosure was issued by Disclosure
Scotland.
PVG Scheme Membership Number,
which is unique to the individual.
PVG Membership Status for the
workforce to which the submitted
application relates. Please note, if
you are barred from the workforce
to which you are applying, you will
not receive a certificate.
Whether or not the individual is
under consideration will be shown
here. When Disclosure Scotland
receives information which has a
bearing on an individual’s suitability
to do regulated work it will be
examined by the Protection Unit to
determine whether or not to put the
individual under consideration for
listing.
