Benedictine Hospital – Information Technology
VPN Agreement
Note: You must have high speed internet access at home in order to install this VPN
software (DSL or Cable). It will not work on modems.
What is a VPN ? Short for virtual private network, a network that is constructed by using public wires to connect
computers. For example, there are a number of systems that enable you to create networks using the Internet as the
medium for transporting data. These systems use encryption and other security mechanisms to ensure that only
authorized users can access the [Benedictine Hospital] network and that the data cannot be intercepted.
By installing a VPN to Benedictine Hospital from my home/business computer, I agree to
the following:
1- ANTI-VIRUS SOFTWARE
Since my PC at home can virus infect Benedictine’s network once I’m connected using a VPN,
I verify that I have anti-virus software installed on my home PC that is accessing the VPN. This
software is periodically updated with the most current anti-virus definitions.
If I do not now own, or in the future discontinue use of my own personal anti-virus software, I
understand that Benedictine Hospital can supply me with “free” licensed anti-virus software for
my PC. I will request such software if I need it, in order to conform to this agreement.
I understand that if at any time – I do not run updated anti-virus software, then I am not to connect
to the Benedictine network using the PC, since I may cause a virus infection on the Benedictine
Hospital Computer network. If I cause a virus to be transferred to the Benedictine Network due to
not running current anti-virus software, I understand that I may be held responsible for any
financial costs and time to eradicate the virus from the Benedictine network.
2- DATA ENCRYPTION
I acknowledge that the VPN software will by default turn on data encryption on my home
computer. In order to conform to HIPAA security policies, I will retain this encryption setting.
3- POLICIES & PROCEDURES
I agree to follow all Benedictine Hospital Information Technology Policies & Procedures
surrounding usercodes & passwords, confidentiality, HIPAA, and security.
4- USE OF VPN SOFTWARE
I will not distribute this software to any unauthorized person or PC. I will remove this software
from my PC(s) if required to do so, or immediately upon terminating employment with
Benedictine. I will not allow unauthorized access using this software, nor share my sign-on codes.

QUESTIONS ABOUT THIS AGREEMENT
If any of the above was unclear before signing this agreement, I have received satisfactory
verbal/written instructions, directions, or information from the Information Technology
department.

Please PRINT and return this completed form to Information Technology department, and you will
be issued a CD containing the software, install instructions, and VPN sign-on codes.
