Position Description Job Title: Clinic Care Coordinator (Patient Advocate) Position Summary: Tasks and Responsibilities: 1. Assists patients navigating the numerous clinic processes and policies for scheduling appointments in medical, dental and behavioral clinics. 2. Facilitate arrangements for various clinic services: transportation, prescription refills, financial aid, energy assistance, safety planning, housing, public assistance, and other services as identified in the clinic resource guide. 3. Based on defined protocols, direct/take patients to an in-house Clinical social worker or behavioral health clinician for case management, intake, or follow-up as directed or needed. 4. Follow up on long or short-term service/treatment plans according to information prepared by a referring provider/clinician. 5. If applicable, provide foreign language translation services for patients who are unable to communicate in English with providers or other clinic staff. 6. Work collaboratively with medical, dental and behavioral health, Human Services departments, the Health Care Home care coordinator, and Hennepin County agencies/departments to coordinate and enhance quality of care. 7. Utilize the Referral Tracking System to provide referral processing, tracking, and followup for patients who have been identified as having additional service needs. 8. Support clinicians in achieving patient centered goals and act as a patient advocate to best serve overall patient needs. 9. Interpret and explain clinic policies, regulations, financial obligations, and programs to clients/patients. 10. Assist patients/clients in preparing necessary forms/paperwork for various services/organizations and agencies. 11. Act as the liaison for external affiliates (i.e. RS Eden, New Perspectives) and coordinates use of established scheduling and reporting procedures between clinic and affiliates. 12. Provides patient advocacy when a patient is having issues/difficulty with any clinic services or processes. 13. Provides back-up for Health Care Home care coordinator. Education and Experience: Bachelor's degree in social work, psychology, sociology, human services, or equivalent field or licensed as a Social Worker by the Minnesota Board of Social Work. No work experience substitutions will be accepted. Knowledge, Abilities, and Skills: Must function well in multi-disciplinary work teams. Must have a good understanding of service integration and its relationship to patient care. Knowledge of: Community resources for adult, pediatric and senior populations; insurance processes, financial aid, housing, food shelves, and transportation resources; Internet based resources; basic computer skills; and Asian or Hispanic cultures is beneficial. Ability to: Establish good working relationships with clinic staff, obtain information by direct or collateral research, work independently, analyze problems presented and develop and carry out effective courses of actions; develop and maintain effective working relationships with other professionals, agencies, and the public; read and interpret insurance benefits and advocate for exception to benefits as needed; communicate well using verbal and written methods; and speak a foreign language preferably Spanish or Hmong is beneficial. Skill in: Using personal computers and automated tracking systems; prioritizing tasks, handling multiple issues while meeting timelines, problem identification and problem solving.
