Clinic Introduction
Our Clinic staff provides simple first aid and emergency treatment for students who become ill or
injured at our school. You will be notified if your child becomes ill or injured at school and needs to
be picked up. It is important that you notify the campus if the child’s medical condition, emergency
contacts, and/or telephone numbers change.
To provide adequate services and to ensure the safety of your student, please maintain the 2014-15
Health History form up-to-date. If your child has a health condition or is currently taking medications
that you would like to discuss with us, please call or come by our Clinic.
We will EXCLUDE students if the any of the following conditions are present:

*Fever – Temperature of 100˚ degrees Fahrenheit or higher
*Vomiting – Witnessed by Staff
*Diarrhea
Influenza (Flu)
Conjunctivitis (Pink-Eye)
Live Head Lice
*Students may return to school after 24 hours sysmptoms free from
fever/vomiting/diarrhea, without the aid of medications.

Confidentiality
A student’s health information is regulated and protected by several laws. Students’ clinic records are
protected under FERPA (Federal Educational Rights and Privacy Act) and HIPPA (Health Insurance
Portability and Accountability Act). We will not leave a specific phone message about the student’s
condition on an answering service or with an emergency contact. We will only ask to return our call as
soon as possible.
