BEAUMONT HOSPITAL BOARD DEPARTMENT OF NURSING: MANAGEMENT OF NURSING In Beaumont Hospital, the Director of Nursing manages and is accountable for the total nursing services at executive level. The Divisional Nurse Manager is accountable to the Director of Nursing for the work of Ward/Departmental clinical nurse managers and clinical nurse specialists assigned to his/her division. The Ward/Departmental Clinical Nurse Managers are in turn accountable to the Divisional Nurse Manager for the performance of Clinical Nurse Managers, Staff Nurses, Student Nurses and ancillary staff working with Nurses on Wards or in Departments. DERMATOLOGY DEPARTMENT Job Title: CNM2 in Dermatological Nursing Grade: Clinical Nurse Manager 2 Hours of Work: 15 hours per Week – Flexible Working Arrangement Contract Duration: 1 Year – Externally Funded Post Reports to: Divisional Nurse Manager – AE/OPD Division Liaises with: Dermatology Consultants for clinical issues Location: Outpatient Department QUALIFICATIONS: 1. To be registered with An Bord Altranais in General Division of the Register. 2. To have name maintained on Live Register with An Bord Altranais. 3. To have at minimum of 5 years post registration experience. Experience in Dermatology Nursing is desirable. A management course is desirable. JOB DESCRIPTION/MAIN RESPONSIBILITIES: JOB SUMMARY: The aim of the Nurse in dermatological nursing is to provide optimal holistic care for patients presenting to the Dermatology Clinic in the Outpatient Department, in-patient wards and Emergency Department as required. The Nurse will also play a pivotal role in meeting the psychological, emotional and informational needs of inpatients, out patients and their families. He/she will be a key member of the interdisciplinary clinical team responsible for skin health promotion. The Nurse should liase closely with all clinicians involved in the diagnosis, management and ongoing care of all patients attending Beaumont Hospital with skin conditions. He/she should be aware of his/her own scope of professional practice (An Bord Altranais 2000a) and recognise the need to refer patients to other specialist services, where relevant in consultation with the consultant dermatologist. The post holder will have a major role in education, acting as a resource to staff in providing professional and academic development, and in the development of quality initiatives within the inter-disciplinary team. DETAILS OF RESPONSIBILITIES CLINICAL COMPONENT: The dermatology nurse will help to meet both the physical, psychological, emotional and informational needs of patients with skin conditions. The nurse will provide this care in the following areas: ✔ Dermatology Department in the Outpatient Clinic ✔ Phototherapy Unit ✔ Renal Transplantation Outpatient Clinic ✔ Emergency Department ✔ Renal Inpatient Wards ✔ General Inpatient Wards ✔ St. Joseph's Hospital, Raheny OUTPATIENT DEPARTMENT The dermatology nurse will give assistance to the multidisciplinary dermatology team and transplant nephrology team using skills within the outpatient setting: ✔ Use a framework of nursing to assess individual patients with skin diseases, consistent with unit policy ✔ Ensures that appropriate nursing care is planned for individual patients based on needs assessment and each individual patient receives the prescribed treatment and nursing care, encompassing medical instructions and in conformance with established standards of care ✔ Ensures that the outcomes of individual care is monitored and evaluated ✔ Ensure that patient confidentiality is maintained at all times ✔ Provide psychological support to patients and families ✔ Develop good communication network with other relevant agencies and clinics ✔ Liaise with medical, nursing and external agencies in relation to individual patients needs and to improve quality of service to patients ✔ To maintain a telephone advice programme for patients currently attending or recently discharged from the dermatology service. ✔ To provide health care information, both written and verbal on all aspects of skin care including eczema, psoriasis, skin cancer, infestations and acne; including information on application of topical preparations and treatments. ✔ To provide information (both written and verbal), support and expert knowledge to patients prior to minor surgery under local anaesthesia. ✔ Coordinate admission of Renal Transplant patients with skin lesions for assessment and treatment as required. This will involve liaison between dermatology, transplant nephrology and surgical teams. ✔ Nurse led clinics ✔ In consultation with the Renal Ambulatory Care Nurses assist a nurse-led clinic which would faciliate a yearly review of all renal transplant patients to faciliate health promotion advice aimed at reducing the risk of the development of skin cancer, skin surveillance, and early referral to the Consultant Dermatology and Surgical Services INPATIENT CARE The dermatology nurse can be contacted by bleep through the internal paging system or by email referral on the internal Nurse Dependency System. The dermatology nurse will: ✔ Follow up renal transplant patients admitted for treatment of skin cancer with a view to providing support to the patient and their family ✔ Assess and advise patients, deciding with the patient the best treatment options. Give advice on the condition of their skin and instruct patients and family in practical skills which need to be carried out after discharge ✔ Advice regarding follow up where appropriate. ✔ Attend dermatology and unit/departmental meetings on a regular basis. ✔ To utilise advanced knowledge, expertise and leadership skills to influence patients continued care with dermatology/multidisciplinary teams. ✔ Ensure strong patient advocacy by maintaining effective communication channels with patient, families and all members of the multidisciplinary team. ✔ Maintain accurate patient clinical records complying with legislation and best practice. ✔ Attend monthly transplant mortality and morbidity meetings when required. ✔ Attend hospital committee meetings as required e.g. accreditation, wound care ✔ Provide an efficient, effective and high quality service, respecting the needs of each patient. ✔ Act as a resource/contact person for any patients with skin conditions who are attending other departments within the hospital ✔ Be aware of existing resources/services, which help patients and their families/significant others, for example: Social services, support groups. ✔ Provide support for patients suffering acute or chronic skin disease. ✔ Provide support for renal transplant patients with skin cancer ✔ Active participation in team discussions regarding treatment for patients with skin conditions. ✔ Involvement in the preparation and implementation of dermatology policies and procedures (An Bord Altranais, 2000c). Also, being aware of all hospital policies and procedures and collaborate with other health care professionals to ensured that these are being observed. ✔ Review current available research literature and facilitate the development of information leaflets for patients and families when required. ✔ Be aware of devices/ aids to enable patients to deal with practical problems related to skin conditions. EDUCATION AND TRAINING PATIENT AND FAMILIES The dermatology nurse will: ✔ Assist in the education on the disease process germane to dermatology prior to discharge or in the dermatology out patient setting. ✔ Assist in the education on medication, both oral and topical related to dermatological conditions and disease prevention ✔ Assess and evaluate each individual patients' needs and learning abilities, taking into account experiences, attitudes and beliefs and provide educational programmes accordingly. ✔ Assist patients in setting realistic goals in relation to the outcome of treatment for chronic skin disease ✔ Facilitate the provision of psychological and practical care for patients, especially those requiring intervention. ✔ Devise, implements and evaluated patient education leaflets in conjunction with the C. N. S. multidisciplinary team ✔ Assist in the education of Renal Transplant patients on their increased risk of developing skin cancer due to immunosuppressive therapy and what they can do to minimise the risk. This includes but is not limited to advice on protection from the sun and instructing patients how to carry out skin surveillance. STAFF The dermatology nurse will: ✔ Accept referrals from the Nursing Dependency System, the hospital intranet and written requests ✔ Educate and empower staff regarding new medical treatment and new products in dermatology. ✔ Empower knowledge by providing formal and informal education and training for general staff nurses, higher diploma students in Nephrology, Emergency and Intensive Care nursing, and other members of the multi-disciplinary team. ✔ Educate, supervise and support junior staff to help them attain a level of competency in dermatological nursing and to enable them to discuss plans and treatment options for patients. ✔ Provide support, guidance and supervision to student nurses, ensuring integration of both clinical and theoretical practice in achieving predetermined learning objectives. ✔ Participate in team meetings and journal club meetings. ✔ Assist in the orientation and provide guidance to new members of the multi-disciplinary team. AUDIT AND RESEARCH The dermatology nurse will: ✔ Participate in regular audit of own clinical practice. ✔ Maintain statistical data within the department when required. ✔ Lead or participate in research as in accordance with the Code of Professional Conduct (An Bord Altranais, 2000b). ✔ Constantly update knowledge and skills and change clinical practice depending on relevant research, ensuring practice is evidence based. ✔ Promote an awareness of research-based practice among staff. ✔ Identify, initiate and participate in research that will benefit patients with skin diseases PROFESSIONAL DEVELOPMENT The dermatology nurse will: ✔ Maintain a professional development portfolio reflecting both continuous professional education and specific speciality education. ✔ Awareness of current trends and strategies in nursing particularly those relevant in providing the highest standard and support to dermatology patients. ✔ Attend relevant national and international conferences relating to dermatology and renal transplantation when required. ✔ Disseminate information gained with colleagues as appropriate.
