Tickets will only be issued to those who meet following the requirements:






Guests must be under the age of 21.
Guests must be a student in good standing at their current High School or a citizen in good
standing within the community.
Guests may not have a criminal record or criminal actions pending.
Guest request form must be completed and approved in order for a ticket to be purchased
A photo copy the guest’s photo identification MUST BE ATTACHED to this form for approval of
this guest form and a ticket to be purchased.
ALL GUESTS must have photo identification in order to enter the venue on prom night.
I have read and understand all rules and procedures for attending SMHS Prom and agree to abide by all South
Mecklenburg High School and CMS guidelines and rules.
