Referral Criteria for Oral Care by the Community Dental Service in
Heywood, Middleton and Rochdale, Bury and Oldham
Referrals are accepted for patients with additional care requirements or that require
specialist care that is unable to be managed by a General Dental Practitioner (GDP).
We will accept referrals for the following:

Patients with a physical disability, wheelchair users who cannot access a GDP as
they are unable to access the dental surgery, and/or transfer to the dental chair
for treatment.

Housebound patients, who do not leave the house for medical appointments or for
social activities (e.g. shopping trips) can be assessed for suitability for
domiciliary care.

Patients with a learning disability or mental health problem and rely on the
support of relatives or carers.

Patients with a sensory impairment, e.g. visual, hearing, and/or communicative.

Patients with medical additional requirements that would not normally be treated
in the general dental services, e.g.
o Oral cancer patients who have undergone head and neck radiotherapy
and may require palliative care.
o Severe bleeding disorder.
o Immunocompromised or immunosuppressed.
o Life limiting condition.
o Brain Injury.

Patients with a severe phobia of dental care and associated co-morbidities, e.g.
learning disabilities, anxious children unable to cope in a General Dental Practice, or
those requiring inhalation sedation.

Children who require specialist dental management, e.g. defective teeth,
missing teeth or abnormally formed teeth, cleft lip and palate, dental trauma,
oral pathology.

Looked After Children/Child In Need.

Patients requiring a second opinion or guidance on their treatment plan.

For Bariatric patients unable to access treatment in a standard dental chair due to
exceeding its weight limit, please contact the service for advice prior to referring
review date 03/16
Please use the referral form supplied. Your referral will be returned to you if
sufficient information is not supplied.
We will gladly give advice over the phone, even if you do not intend to refer the
patient but would like advice about a patient with special needs.
The above criteria are not exclusive and each case will be judged on its own merits.
