Registration form and essential information:
The conference, accommodation and food, will be held in the Kaskady hotel, Sliac, Slovak Republic. The delegate registration fee includes: the participation in lectures,
proceedings, simultaneous interpreting between Czech and English, gala dinner, food. The accommodation, beverages, mini bar and the others services of hotel are not
included in the registration fee. The price of accommodation per night and room is: single room EUR 91 – 121; double room EUR 118 - 162; suite EUR 169 and it includes
breakfast (6.30 a.m. – 10.00 a.m.), parking, thermal pool, sauna, fitness and the internet. Hotel check-in is possible from 02.00 p.m. and check-out has to take place by
10.00 a.m. Wellness and sport services are available at www.hotelkaskady.sk. If you wish to book any wellness procedures, contact the wellness reception at tel. number
+421 455 300 004. Accompanying person is considered to be a spouse or a common-law wife/husband. The title of the lecture and the name of the speaker shall be
communicated by 30.6.09 and the registration form shall be sent by 30.9.09 to the CSGA office via E-mail, fax or post. The payment of the registration fee by the
participants located outside the Czech Republic shall be made cash in the registration at the hotel or in advance by bank transfer including the required bank charges.
