Overview:
Ealing Park Health Centre is a young, dynamic, forward thinking and developing
practice of 9,250 patients in a socially and culturally diverse area of Ealing. As well
as offering the full range of standard services, we are growing the range of enhanced
services delivered. We are a teaching practice and enjoy a lively and engaging
learning environment in which all members of the practice are encouraged to
participate. We are fully involved in shaping our role in the local health network and
more widely at commissioning consortium level and we strive to be able to continue
to develop the quality of care for our patients in the current changing landscape of
the NHS.
We have an opportunity to establish a first class practice nurse team of a nurse
practitioner, a part-time nurse and a healthcare assistant to fully integrate into the
existing clinical team of 4 partners and 2 salaried doctors.
Job summary:
The successful candidate will be a team player with excellent communication skills
and will:
o take responsibility for delivering primary nursing care including aspects of
chronic disease management involving monitoring and patient education to
promote self management.
o take responsibility for delivering screening and public health programmes
associated with primary care.
o contribute to the clinical management in terms of the organisation of the
clinical work with the clinical team.
o take initiative in developing and delivering new services as opportunities
arise in line with surgery vision and need.
o participate in the clinical meetings including leading the learning on occasion.
o support the training of F2 doctors.
o liaise with outside agencies such as health, social and voluntary agencies,
and other surgeries in our health network and participate in the local nurse
forum

Main responsibilities:
Clinical












Good quality Practice Nurse skills including immunisations, travel vaccinations,
wound and ulcer care, injections, phlebotomy/ paediatric phlebotomy, cervical
smears, obtaining high vaginal swab taking, ear syringing, contraceptive care,
management of clinical emergencies within the surgery.
Chronic Disease Management for diabetes, COPD, asthma, essential
hypertension, heart disease and mental health following agreed protocols to
promote patient self management. This will include patient education and
monitoring.
ECGs and 24hour ambulatory BP monitoring
INR monitoring using INR STAR
Cryotherapy and assistance with minor surgery procedures
LESs and DESs as appropriate
Involvement in the Integrated Care Pilot- completing care plans for identified high
risk cases
Maintaining the emergency drugs and equipment
QOF
Audits
Public health campaigns within primary care
Good communications with other team members when clinical findings on routine
examination indicate possible action/follow up.

Infection Control
Infection control is everyone’s responsibility. The post holder will adhere to and
promote within the team the practice’s Infection Prevention and Control Policies and
make every effort to maintain high standards of infection control at all times, thereby
reduce the burden of healthcare associated infections including MRSA. In particular
all staff should have the following key responsibilities:
 Staff must wash their hands or use alcohol hand rub before commencing
clinic and between each patient contact.
 Staff have a duty to attend infection control training.
 Post holder will take lead in screening surgery staff should take lead in
screening surgery staff should an infection be detected or reported within the
clinical team.

safety comes first and to treat patients, carers, family members and colleagues with
dignity and respect.
