Please state your charitable status and number. 8. The name of the organisation to which the grant should be paid (account name). 9. The up-to-date bank details of your organisation (name and address of bank/building society, account number and sort code). 10. Proposed start and completion dates. 11. Grant requested from CAFOD (£0.00). 12. Signature and Date Your application must be signed by either the Chair of the management committee or support group or the Director/leader (as applicable) to indicate support for the project/programme. This is a requirement for reporting and accounting procedures within CAFOD. CAFOD will not accept applications signed by individuals who are direct beneficiaries of the grant). B. C.
