AGREEMENT FOR USING DSS TEST PROCTORING SERVICES University of Alaska Anchorage students who use the test proctoring services of Disability Support Services (DSS) enter into this agreement to ensure the quality and continuation of those services. Student agrees to: 1. Keep all test contents and materials confidential. 2. Complete the test at one sitting. 3. Use only those materials noted by the professor on the Test Accommodations Form on file at DSS. All other materials such as backpacks, books, etc., must be left with DSS staff during testing. 4. Schedule tests during the same day and time the rest of the class is testing. Alternative arrangements may be made if the class meets outside of DSS operating hours, extended time creates a time conflict with another class, or other extenuating circumstances. Alternate arrangements are made on an individualized case-by-case determination. 5. Appear at the scheduled start time. The total testing time will be calculated from the scheduled start time. For example, a test is scheduled to begin at 12:00 noon, with two and a half hours for testing. Testing will be stopped at 2:30, even if you show up to start the test at 1:30. 6. Schedule tests a minimum of two (2) days in advance. Notes: If you are more than two (2) hours late for a scheduled test, the test will be returned to the professor. Test rescheduling is not permitted without written or verbal permission given by the professor directly to DSS. Failure to follow the responsibilities listed above may result in the suspension of test proctoring services by Disability Support Services.
